Airway clearance and how I feel throughout the day

Posted by @ling @wangling, May 4, 2023

Hi everyone,
I know it might be a diffuse question, but let me try.
My pulmonologist told me if I do the airway clearance correctly. I
should feel clear of mucus during the day. I spend almost 30 minutes
doing it (flutter + autogenic drainage in the morning and evening). But throughout the day, I have to constantly clear out mucus most days. I am wondering what your experience is, is it normal for you? Maybe I am too sensitive about it.
Your experience is greatly appreciated.
Many thanks!
Ling

Are you nebulizing twice a day with 7% saline?

Only 3 percent preceded by albuterol neb
I have started doing all together along with my oscillating vest through the aerobika
So I get more time extracting mucus
My pulmo reduced my AC to once a day because I was getting very exhausted, particularly using the aerobic to the point where extract most of the mucus
If I do it for too long, I don’t have enough energy to do a good fast walk/workout

Ling I hope this helps answer your Question
Maybe you should try the nebulizer before you do the C
I find that if I do them all together, along with the oscillating vest to the point where my huffs don’t wheeze or rattle, I’m Mostly clear for the rest of the day and have good energy

Hi Jill.
My main symptom is coughing up gunk all day long and overnight. Things are especially challenging in the morning due to mucus accumulation while I sleep. Airway clearance usually only buys me about 15-20 minutes of peace before the gunk factory kicks back in. It’s not much fun, as you might imagine. Contributing factors include BE, frequent infections including four or five lung bugs since last September, scarring and blebs and bullea, and a 5 cm cavitary lesion in my left lung. Six months of antibiotics for MAC did not reduce the coughing or mucus hypersecretion. I also take NAC and Mucinex every day. Most of the issues are in my upper lungs. One of my docs suggested meeting with a thoracic surgeon, while also noting that I might not be a good candidate for surgery due to low BMI and other factors. We also discussed palliative care. My current game plan is to stay aggressive with airway clearance, and I will probably be restarting antibiotics since a recent sputum test was positive for MAC. As always, hoping things turn around in my favor. Doing lots of research, taking various supplements, and trying to put on a few pounds. Forward on, as they say. I hope your health situation improves.

My situation is the same as " my main symptom is coughing up phleghm all day long and overnight" , and especially in the mornings. I have been on medications for MAC and they cause me more adverse reactions than do any long term benefit. Trying to maintain employment is the most troublesome thing , in the context of doing nebs, airway clearance, in an attempt to keep infections at bay. I hope everyone's health improves and I will keep in touch with this blog as it helps keep me grounded.

I fully understand. I am so exhausted by the time I finish albuterol and 7% saline with vest followed by flutter valve, autogenic drainage and then positional drainage I feel like laying down. Takes over an hour. At bedtime I do nebulizer, flutter valve, autogenic drainage and head to bed thankfully.

Hi guys,
I recently started Arikayce and found out the Saline helps for clearance. How do you disinfect your nabulizer for saline use? My pulmonologist's nurse said to just wash it with soapy water. It doesn't make sense in light of the fact that the Arikayce nabulizer is sterelized. I would appreciate your input.

After a soapy soak, some people boil their equipment for 10 minutes every day or even after each use. Some boil once a week. Some sterilize using a baby bottle sterilizer. Even though we all aim to accomplish the same thing -- clean and sterilized equipment -- we each manage slightly differently. Really depends on one's disease state and risk tolerance. Since there is no home test for swabbing our equipment to find out if our method works, we look to experts like Dr. Falkinham, who published papers on NTM and its sources. His papers can be found in the ABC's on bronchiectasis thread. So you can decide what's right for you.

For me, it's a baby bottle sterilizer, Bololo, a few times a week for equipment sterilization. I have bronchiectasis without NTM (so far, working to keep it that way with 2x day clearance etc etc etc). Remember that sterilization of nebulizers etc is only one part of the equation for NTM, pseudomonas, etc avoidance.
