I am interested in what everyone's numbers were at time of diagnosis of Smoldering Multiple Myeloma SMM? Were you Kappa or Lamba?
kappa free light chain in(mg/L):,
K/L Ratio(mg/L,):,
M-Spike(g/dL):

If Lambda
Free Lambda Lt Chains,S (mg/L):

I am newly diagnosed with SMM('26) (was IGG Kappa MGUS '20)and wanted to get an idea as some say my Kappa and ratio numbers are high. what other numbers do you track?
Free Kappa Lt Chains,S (mg/L): 112
K/L Ratio(mg/L,): 10
M-spike 1.4

Just for comparison:
I’ve had IGA Kappa MGUS for at least 11 years.. My last results were the following:

Free Kappa Light Chains (mg/L): 90.5 Range 3.3-19.4
Free Lambda Light Chains (mg/L): 11,7 Range 5.7-26.3
K/L Ratio: 7.71 Range 0.2-1.65
M-spike 0.6 g/dl

I was told that I am still at the MGUS stage and that I have not moved to Smoldering yet.

I am also interested in what new results would be necessary to show I had moved to the Smoldering category.

I am still Light Chain SMM and my Kappa levels were:
Kappa: 89 mg/l
Lambda: .84
Ratio: 101

There is no m-spike with Light Chain SMM.
My numbers have gone down a little since I was diagnosed (without treatment). I am on a watch and wait schedule where my bloodwork is tested every two months for now.

I have IgA Kappa Free Light Chain SMM. One can have an M spike with LC SMM; mine is 1 (near zero a year ago). I was diagnosed with MGUS 10 years ago and my numbers started ramping up 3 years ago (SMM last year). I'm high risk due to SMM type and 1q21 gain, so on the 2 mo bloodwork plan. If/when I reach a threshold, I will start I-VRD treatment. SMM can ramp up quickly, can also plateau, and rarely can subside some. The waiting is hard. My numbers are:

Kappa 932 mg/L (378 six months before and still MGUS)
Kappa/Lambda ratio 75 (32 six months before, MGUS)
M Spike 1 (6 mo before near zero)
BMB 9/25 had 15% deranged plasma cells (10% is usually SMM, but hematologist still thought MGUS due to bloodwork)
