Can anyone tell me if medical marijuana is permitted pre-transplant?

Posted by feruj @feruj, Dec 15, 2022

Hi, I am new to the forum and glad to have found support since I really haven't shared with any friends or family.
I have liver disease (horrible nausea, headaches & sleepless), and am in the process of meeting with a transplant team. Can anyone tell me if medical marijuana is permitted pre-transplant?

@feruj, I'd like to add my welcome. Medicinal use of marijuana has been shown to help some people with the symptoms of nausea, headache and sleeplessness. I can understand you wish to see if it would provide some relief for you. However, you are equally wise to research whether the use of medical marijuana might jeopardize your chance of being listed or receiving a liver transplant.

The answer is not clear. You may wish to read the medical and ethical complexities in this paper:
- Denying renal transplantation to an adolescent medical cannabis user: An ethical case study https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6671627/

My recommendation would be to talk with your transplant social worker before using marijuana at the hospital where you are listed. They may have criteria specific to the hospital's transplant listing. Be honest and they will work with you, either to use marijuana safely or to find alternatives for your symptoms. I truly hope you find relief.

And in jacksonville we came to a reasonable conclusion compromise. I ceased my medical cannabis and started on trazidone and extra strength otc at bedtime. This achieved about a 50 % effective rate and it beats being denied. Hth

@feruj - Definitely ask your transplant team. I think it depends on the State and the hospital. I wish I would have asked. If you ask and the answer is yes, I would get it in writing. I remember pre transplant they tested for everything from alcohol, marijuana, barbiturates, etc. I did ask Post Transplant to use a cbd/thc gummie and the answer was yes.
More and more I think the medical industry is coming around to the benefits of thc.

I currently being monitored (not listed yet) by Mayo Jax for possible Lung Transplant in the future. I have NSIP with fibrosis, plus Bronchiectasis. I have a hard time sleeping at night due to the amount of mucus that I get in my airways from the Bronchiectasis. Does Does anyone know if Mayo Jax allows patients to try CBD Gummies only!!!! To help with sleeping. No THC in the gummy. I asked my social worker and she wasn’t sure, wrote to my transplant coordinator but haven’t heard back yet. Didn’t know if anyone has had this experience. Thank -you.

Hello @spider109,

You bring up a great question that has a really tough answer that does not seem to fit all. Members have talked about cannabis and transplant in the following discussion, https://connect.mayoclinic.org/discussion/smoking-cannabis-post-transplant/, although that discussion focuses on smoking cannabis vs. CBD gummies without THC.

It may be a cliche, but you have taken the right step in reaching out to your transplant coordinator. Because medications and supplements have such a vast array of contraindications, it is smart to wait for guidance.

@athenalee and @seventhson89 discussed CBD in the above discussion and may have some updated experiences to share with you. Another over the counter option would something like a melatonin, but you'd still need to converse with your transplant coordinator to check for any possible drug interactions.

My medical cannabis doctor gave me some information I did not like, but had to swallow anyway. Cannabis and Coumadin do not play well together. At the start of my work ups I had to stop all medical cannabis. This medicine is from vetted, state licensed dispensary. Post transplant she had no experience with, but did advise to abstain until more facts were available regarding immunotherapy drug contraindications . Many other details on this and my experience. As stated above, your primary care at Jax and your medical cannabis doctor are the best resources. My delivery route was oral. Smoking it is too hard on my lungs. We make our own ointments from thc oils . Edibles are purchased at licensed dispensary. RSO is what was taken. Too much information to share here, but follow your medical professionals advise. Hth

I live in California and I'm traveling to Phoenix in early June for a Kidney Transplant evaluation. I've multi listed at Mayo and UCSF. Question. I use occasional marijuana gummies 10mg for pain and sleep. I see there is a urinalysis test are they looking for marijuana. Its legal in both states. I haven't had one in about 10 days. I'm not a heavy user. I don't want to be disqualified or have to go to some detox thing. I'm not addicted wouldn't make any sense for me.

If legal I wouldn’t worry. It shows on my transplant records ‘occasional user’ and no-one cares.

@slavalle,
I would assume that you have already listed this usage in the medical records that you have sent in preparation for your visit/evaluation.
With many meds (over counter and prescription) and suplements there is a risk of damage to the transplanted organ that is not a concern for the general population or the legality of a product - there are individual differences with every patient, plus, there are unique concerns when an organ disease, failure, and transplant is involved.
I strongly suggest that you communicate with the transplant dept before your visit. Does that sound like something you could do?
