With your PSA bouncing erratically, although no overly high, i was in a similar position. My PSA went from it’s annual stability of years of 1.9 as follows over the next three years:
3.0, 2.5, 2.3, 3.2, 4.0, 2.9, 3.4, 4.1, 3.0, 3.3. Initially i was told my PSA was consistent with my age until I went back gathering all the data and talked them into the MRI. My initial MRI labeled my findings suggestive if subacute on chronic prostatitis, so we just continued the PSA testing quarterly. Also initially with those first couple drops took the anxiety away. Just getting really concerned again hitting 4., but dropped again. It was my primary doctor not my urologist who knew what was happening and my concern, told me his father was in a similar situation regarding getting a biopsy and they obtained the ExoDx test., so he was the one that actually ordered it for me and it came back 72.56. This then got me another MRI 18 months after my first one with the PIRADS 4 lesion, although that specific lesion benign found cancer elsewhere from my biopsy and as they say the rest is history. I think moving forward with your biopsy is good on July 6th and just enjoy July 4th!

While the information you have sounds negative. There’s really no way of knowing anything until you get a biopsy.

PIRADS 4 Doesn’t definitely mean you have prostate cancer or that it’s aggressive. The ExoDx Score is high, But you don’t know what your biopsy shows yet so it’s really not possible to guess what’s going on. A high score does not automatically guarantee that cancer is present, but it strongly suggests that further investigation is warranted

A 77 could mean you have a chance of a Gleason seven or higher, not that you have something that’s not treatable. You could live for decades. You just need more information.

Definitely not time to panic since prostate cancer is a chronic disease for almost all of us these days.

I think a biopsy is definitely a good next step. The ExoDx test is not one of the most accurate any more as the technology has advanced, but it can give a good indication of the presence of cancer.

I had an ExoDx test and the results were elevated (I think the score was 35) which led to an MRI which was completely clear and then a saturation biopsy. The saturation biopsy showed two cores that had a small amount of cancer that was graded as 4+5, and led to a RALP six weeks later. The final pathology report after the RALP downgraded my final Gleason score to 4+3 with tertiary amount of 5.

I would not have been diagnosed as quickly as I was without the ExoDx test so I am a big believer in these newer tests that are more definitive than the PSA test. It's good that you had the test, and given the results I agree that a biopsy is a good next step. I would suggest the saturation biopsy (typically 24 cores) to make sure that all parts of the prostate gland are sampled.

What was your initial psa and did you have to go on adt therapy and how are you doing now? Just hoping for a decent Gleason score but I guess if it's contained even if it's higher it can be managed with the right treatment. If given the choice I will probably opt for prostate removal. Thanks for responding.
