ExoDx test: What scores are worrisome?

Recently I had an mri for an psa of 5, they found an .8 cm wedge lesion and said fi di gs consistent with prostatitis and gave it a pirads 4 rating. He wanted to do a exodx test to determine whether to proceed to a biopsy. What scores are worrisome and how accurate are they in predicting aggressive cancers. Say if you get a 30 score does that mean higher probability of higher Gleason score than 7.

I think a biopsy is definitely a good next step. The ExoDx test is not one of the most accurate any more as the technology has advanced, but it can give a good indication of the presence of cancer.

I had an ExoDx test and the results were elevated (I think the score was 35) which led to an MRI which was completely clear and then a saturation biopsy. The saturation biopsy showed two cores that had a small amount of cancer that was graded as 4+5, and led to a RALP six weeks later. The final pathology report after the RALP downgraded my final Gleason score to 4+3 with tertiary amount of 5.

I would not have been diagnosed as quickly as I was without the ExoDx test so I am a big believer in these newer tests that are more definitive than the PSA test. It's good that you had the test, and given the results I agree that a biopsy is a good next step. I would suggest the saturation biopsy (typically 24 cores) to make sure that all parts of the prostate gland are sampled.

@rider51 as i understand it, the exodx is very good fornegative predictabilty, that is if it comes out negative, it is something in the 90's% correct. However, its positive predictability is not so good. In other words, it is used more to rule out cancer than to confirm its presence.

Re recently mri a .8cm lesion wedge shaped and linear rated pirads 4 t2 with a dwi of 3, and findings noted consistent with prostate Infection ut my exods score came in at 77 which was very concerning biopsy scheduled for July 6th any thoughts?

My mri noted findings consistent with prostatitis that's why I had the test, but also read prostatitis infection can skew the exodos numbers, so I will wait for the biopsy results and see, also the .8cm lesion is wedge shape and linear which can be indicator of prostatitis, but they also can be sometimes malignant also, will see what happens with results from July 6th I will let you know, thanks for responding.
