What could be causing my constant feeling of head pressure?

I have a constant feeling of head pressure 24/7 that has lasted for months. The pressure is all over my head except the back. It improves a bit when laying down. I also have dizziness, brain fog, and severe fatigue probably related to this. Brain MRI scans show no issues. Has anyone had this issue, and what was the cause and solution?

I have a pressure (not painful) left side of head...sometimes behind left eye, sometimes on top left side.
Not painful.
No dizziness
No inbalance
Vision ok
I had a CT scan, no problem.
Getting MRI in a week.
I am lucky to live 2 miles from Duke Univ. Hospital.
Lots of big wigs there.
BTW my neck stiffens up when I try to look left.
I will get back with you.

I would see a neurologist for this. Could be migraines without aura.

I am an active 88 year old woman, with no other physical problems. In June of ‘24 I suffered a concussion, hitting my forehead. Symptoms which included icepick headaches subsided after two weeks. After 3 weeks I caught Covid, recovered normally. Head pressure began 4 months after my fall and has continued to this day. There is no pain, only pressure.

I have seen 3 Neurologists, taken numerous medications, 3 CT scans and several MRIs, 2 treatments of therapeutic Botox.

Pressure started from the site of the fall (top of right forehead)and is progressing down towards my eyes. I can no longer read as it irritates both my eyes. There has no relief, or no diagnosis as to the cause of the pressure. Any suggestions would be greatly appreciated.

barriefahey, i would want imaging for a subdural hematoma specifically an MRI, though generally they like to start with a CT, given the history of the fall. It could advantage you to have a full eye exam; that might yield clues to the pressure. But it is also important to protect your vision.
Inadequate sinus drainage can cause this kind of head pressure and also cause pressure on the optic nerve. This pressure can damage the optic nerve. The fall may have caused damage to the ostia the small drainage passages in the sinuses. A good primary care can order a sinus CT and brain MRI; some prefer to refer you to a specialist.
Wonderful to be in such good health. I hope this issue is solved readily.

@gently I have had 3 cat scans and 4 mris. I have also have had two complete eye exams in the last four months. Have had no problems with sinuses,bu eyes are getting worse. Have been trying to find some help for over a year. Have had two therapeutic Botox treatments with no result. This was done by a neurologist.. how should I proceed from here. Concerned that it is getting worse

@barriefahey - just wanted you to know I moved your post here so you could talk with others in this existing discussion about head pressure. Hoping others here will have some input for you on this pressure that began 4 months after your fall resulting in a concussion and that continues now, making it impossible to read, with no relief or diagnosis.

I'm guessing the numerous medications you've been prescribed (or taken over the counter) have not been helpful, barriefahey, nor the (Botulinum Toxin Type A) Botox shots? Did any of the scans reveal anything useful? Did the neurologists you saw think the pressure could be related to the COVID-19 you had soon after your concussion, or did they think that was unrelated?

were the eye exams with ophthalmologists or optometrists.
You've manage to get a good number of MRIs 4. If they are recent you might try for a Optical Coherence Tomography (OCT). It sounds like your care is very accessible.
What do the eye exam doctors say about your vision.
You might get a CT of your sinuses, usually easy to get in a allergy office, because even if you are breathing just fine, you could have sinuses that are completely blocked. Sometimes with cysts but sometimes (I know a case where) the sinus is totally filled with dormant infection.
I'm really thinking of the frontal sinuses because that's where you were impacted.
It might be possible that this is an unrecognized, long covid symptom I'd guess not.

I'm sorry you have this head pressure. Has anyone suggested Long Covid? I have Long Covid and have had head pressure all over my head for almost six years, mostly the back, neck, and top of head, but sometimes the face. I had probable covid in March 2020. MRIs are fine, except two neuroquant MRIs show parts of my brain has shrunk (probably from covid and maybe age, I'm 65). My headache neurologist has given me the list of medicines for migraines, nothing has worked. I had Botox shots last week--will see if that helps. Please take care and I hope you get some relief?
