Kidney Transplant; MRI with Contrast

Posted by hello1234 @hello1234, Oct 18, 2022

Hi all,
I am a two year post kidney transplant patient. During an abdomen ultrasound, a "complex cyst" was found on one of my native kidneys. They would like me to have an MRI with contrast to better diagnose the complex cyst.
I currently have BK virus that is improving, but not resolved. The ultrasound showed mild calieltasis on my new kidney which shows irritation (I assume from the BK virus).
I am scheduled for the MRI contrast next week but now I am concerned about stressing my new kidney even more on top of the virus. What are your thoughts?
Thanks everyone!

I would ask your transplant team.
I am almost 2 years post transplant and did have a CT with contrast in ER this summer and Mayo told them to give IV hydration.

Thank you @jennifer0726 😊
I am also a two year post kidney transplant patient. It's very helpful to know that you proceeded with contrast and that IV hydration was used. It sounds like everything went well. Thank you for letting me know your experience with contrast!

I have been told to never allow contrast and so that is what I have done since my kidney transplant. When a hospitalist ordered a MRI or CAT scan (sorry can't remember which one) for me last year with contrast, I allowed the test but refused the contrast. They were still able to make the diagnosis. I know that the dye is difficult on the kidney. I would highly suggest you speak to your transplant team, before allowing them to use contrast.

Hi @hello1234
While I am a heart transplant recipient, I am allergic to the traditional contrast dye and it does damage my kidneys. The last time I had an MRI I was given a different type of contrast – gadolinium – which is not iodine based. Perhaps you could ask your team about this possible alternative.
Let us know how it goes.

Thank you @estrada53 😊 That's very helpful information. I will definitely write that contrast name down and ask about it! I want to protect my transplanted kidney as much as I can. Now I am mad at myself for getting covid! I was scheduled for an Evusheld shot and I postponed it when I got the BK virus thinking I may be adding to much stuff to my kidney. But that was stupid, because getting covid is definitely a greater risk then taking Evusheld. I find it difficult to weigh all the risks and rewards of every decision.
