Protect Your Vaccine Record Card

I was just looking at my half-finished vaccination card. Next Sunday is my second vaccination. I can't wait. It's been well over a year that I've been out and about as most of us.

But hold on, what do I do with my card once I'm vaccinated? CNN has some wonderful tips.

First off make sure that all the information on the card is 100% accurate BEFORE you leave where you are getting vaccinated. (name, date of birth, and the date and location of the vaccine -- for accuracy. )

Secondly, take a photo of both sides on your phone, for the first vaccination (if possible) and then the second. Keep a copy on your phone and email a copy to yourself. Then photocopy it and keep at least one copy with your important documents.

Lost cards or very damaged ones need to be replaced. Contact the provider of your vaccinations if you need to replace yours. You might want to jot down the time and place of your vaccination in case you lose your card and need to replace it. Visit your health department immunization information system (IIS). My state has a COVID-19 Vaccine Portal through the State Department of Health.

What is a COVID-19 vaccine card?
The card was created by the CDC to be a helpful reminder and a way to confirm that the correct second dose is provided to you.

At Mayo Clinic, however, the card is not necessary because Mayo is able to track when you should return and which vaccine you will need for your second dose.

Should you have a need for the information on the card, it will be made available in your Patient Online Services account under immunizations.

Thanks for the response. Some vaccine providers are issuing cards that
certify that the vaccine was administered to the card holder. These may be
required at travel check points or for access to other areas which have
entry restrictions. I just read that Mayo is working with Oracle and other
tech companies to develop a card which will contain medical records,
including vaccination history. My wife got her vaccine at Ascension St
Vincents - they provide a document that shows the vaccine was administered.

@amandajro and @flcipriano44, in addition to what Amanda shared, Mayo is not in my area and the clinic nurse told me at my 2nd vacc to hold onto the card issued in case it might be required later for some types of travel. It is small and tucks easily into a billfold.

@merpreb Good ideas, Merry. But, I don't remember getting any card when I was first vaccinated at Mayo in Jan. I don't like the idea of a gov't or any mandated covid vaccination card, but think it's good for my records and if I ever need it.

I also think it would be a good idea for us to keep records of all vaccinations.....my med records w/PCP and Mayo have everything they know about. Wonder if most folks, children going to school, have such records? I remember having to show proof of vaccinations the first day of school every year, or update the records. Bet that's not done anymore and maybe should be since we've had resurgence of measles. It can be quite dangerous and spread like crazy, even more polio cases recently I think.

@ess77- To my knowledge everyone who has been vaccinated needs to be given a card with dates of your shots and your name and address. Could you have been given one and not payed attention to it?

If I were you I'd call the provider and find out about this. These cards are the only thing that we have that documents our vaccines.

Merry, You are right...everyone gets a vaccination record card with the CDC logo on it. You bring the card back if you are getting the second shot. @ess77 Try to get a electronic or paper record of your vaccination then take a picture of it and keep it on your phone. All my grandkids records over the years are both paper and electronic. Even our 4 legged friends need proof of vaccinations if they are being boarded.

What has anyone heard about laminating the vaccination card?

Office Depot and Office Max and , I think Staples , will laminate your fax card for free. Amazon is already selling card holders ...paper and plastic. I put a copy on my phone and just filed my original away. I made copies of my SS and Ins card at home on a piece of laminated type photo paper ...cut them to size and put in my wallet. Originals just filed away.
