@ess77- To my knowledge everyone who has been vaccinated needs to be given a card with dates of your shots and your name and address. Could you have been given one and not payed attention to it?

If I were you I'd call the provider and find out about this. These cards are the only thing that we have that documents our vaccines.

Merry, You are right...everyone gets a vaccination record card with the CDC logo on it. You bring the card back if you are getting the second shot. @ess77 Try to get a electronic or paper record of your vaccination then take a picture of it and keep it on your phone. All my grandkids records over the years are both paper and electronic. Even our 4 legged friends need proof of vaccinations if they are being boarded.
