After transplant I have hair loss due to meds: Will it come back?

Posted by jtjourney @jtjourney, Jan 30, 2025

I had my liver transplant 7 months ago. Everything went wonderfully, however, I'm experiencing severe hair loss due to my anti-rejection meds. Will my hair come back? Is there anything folks have done to help with the hair loss? I'm taking Biotin for several months and it hasn't helped.

It took around 1 year for me to feel that my hair and finger nails had reached a satisfactory level.
I took biotin as directed by dermatologist, with transplant team approval. And I used gentle hair products that my hair dresser uses for her cancer patients.
My hair health has continued to improve over time! As I approach my 16th year with liver/kidney transplant, my hair is shiny natural silver (genetic and vitiligo) and my nails are clear, hard and healthy!

By chance do you have any specific recommendations? My mom had her liver transplant earlier this year in March and it hurts to see her having such a hard time with losing hair. She ordered some natural hair growth shampoo on amazon that is meant for all hair types so I hope it helps or becomes better over time. Let me know!

@elijunkk310, I would like to extend my welcome to you. I am happy to hear that your mom has received a liver transplant and is to the recovery stage that I remember happened to me, when I was feeling well enough to be concerned about my hair! I am fortunate to have a hair dresser who caters to cancer patients (hair care and custom wigs). She suggested that I use some of the gentle product (shampoo and conditionsr) that she recommends to her cancer partients. And then at my post transplant check-up, I was told that I could try Biotin, that it might help. But I was assured the my hair would come back in time - It did.

I assure you that your mom is not alone! Here is a link to more members talikng about hair loss:

Does your mom have a dermatologist? I would suggest that she ask the dermatologist or post transplant nurse about the product that she has ordered online.

Other than hair, How is your mom getting along with her recovery?

I had hair loss also after my liver transplant. It's quite normal. Talk to her doctors about Biotin. It does work, but also in time the hair loss will stop. Good luck with everything and God bless.

I am on tacromilis and had hair loss the first year and once in a while through the last six years. I tried a hair loss shampoo like you mentioned and I believe it works. I stopped using it for a bit and noticed some hair loss returned. I have very thick hair and I noticed I could see my scalp in some areas. I was surprised how much it bothered me. I had chemo over 12 years ago and lost all my hair - I guess I expected it then. When I lost hair from the tacromilis I kept asking my family and friends do you see my scalp ? They tried to be nice and said no but finally someone said yes. That honesty helped me figure out ways to address it. Here’s to recovery for all.

I'm pre-transplant and my hair started falling out about three months ago. The dermatologist who is very up to date on medical conditions said mine was due to pressure on the hepatic vein with ascites. She said to use Minoxidil 5% foam that I got from Amazon but it made me itch. The two other ingestible products were nixed by my hepatologist as they would make my liver condition worse. I had already been taking Natures Bounty Hair, Skin and Nails but switched to the new advanced formula of the same product for the last few months. I just waited to see what was going to happen and it has stopped falling out and is now attempting to grow back. What hair didn't fall out has grown 1/2 inch so I think I'm through it. Fingers crossed.

Im 8 months post my dermatologist said that my body was recovering from trauma ( the surgery) and that's the first thing your body does is stop growing hair, and your fingernails are probably very brittle as well. That was my experience.Mine's finally started to grow back after about 4 months

I have hair loss since my transplant 3 1/2 years ago. I take envarsus and myfortic. I am seeing my transplant doctor tomorrow to discuss making a change to either cyclosporine or belatacept. Both have scary side effects but I want my hair back to normal.

Since you posted yesterday, I am assuming that you are seeing your transplant doctor today. I hope you have a good visit with good labs and no surprises.
As for the hair loss, I also experienced hair loss in the beginning after my transplant surgery in 2009. It was the dermatologist who suggested that I try Biotin. For me it was an easy thing to do and approved by my transplant team. There might be some newer approved solutions now. I hope that you are careful with products that you put on your hair.

I am surprised that your hair loss has been ongoing for 3 1/2 years. I hope that you can have a meaningful conversation with the doctor(s) and learn what might be the cause for the hair loss.

-related to the reference to Tacrolimus in the title: I have been taking Tacrolimus for 17 years. Each of is is different and alike!

Thanks so much for your response. I tried biotin and it didn’t stop the shedding. I have tied every serum including minoxidil and none helped. I have been using Trybello spray and it has helped. I’m not switching to another immuno drug. I will stay on Envarsus as I read last night that lowering the Envarsus dose after 3 1/2 years with my GFR of 94 and creatinine of 0.60, shouldn’t be an issue. I take 3.5 mg each day and my trough level as of yesterday’s testing is 7.1. The info I read stated a reduction of trough level to between 5.0 - 6.0 can eliminate side effects of hair loss, etc. So I texted my doctor with this info and hope she lowers my dose.

Thanks again for your uplifting text.
Sincerely,
Barbara
