People seek driver evaluation after strokes or head injuries, as well as for dementia or cognitive impairment. You should be able to do a simple internet search for driver evaluation near you.

Few states link results of driver evaluation to driving privileges; you can flunk and still keep driving legally, though you may be encouraged to surrender your license. However, a physician can notify the state or county that issues the license and recommend the patient's license be withdrawn.
