HIGH TSH AND HIGH T4

Hello All,

7 months ago September 2025, I noticed my throat was enlarged and that's when I got an ultrasound on my thyroid. The findings where the texture was heterogenous, left lobe was increased 5.1 x 1.5 x 2.7cm and right lobe was 5.3 x 1.7 x 3.3cm. no nodules were noted.

I got a thyroid panel done at the same time sept 2025, I found that my T4 was 2.17 ng/dl and my TSH was 15.227 ulu/ml, T3 is normal, my thyroid peroxidase AB is 600 iu/ml and thyroglobulin is 4,000 Ui/ml. Since sept 2025- Mar 5 2026 my TSH and T4 are both consistantly high.

So I was referred to an endocrinologist and I mentioned if this had to do with my pituitary gland so at first he was saying no because my prolactin levels were good but a couple days later he called and put in an order for me to get one. The results were that pretty much everything was normal except my pituitary gland was midly prominent in size. thats all.

I am and have been experiencing all the symptoms of hypo and hyper, waking up feeling hungover, heart racing and heart fluttering, headaches, foggy vision, brain fog, bone pain, along with back pain, heat and cold intolerance extreme fatigue and dry eyes. My next appointment is later this month.

Id like to know your thoughts and what I shouldnt except for treatment and what ideas you have that should be ruled out. I have felt like death for 2 years now and only 7 months ago im getting an idea on why.

I am 24yr FM.
No kids
Never been on hormone medication
I don't take Biotin

I was diagnosed with hypothyroidism and Hashimoto's years ago when receiving similar blood tests. Have they checked for Hashimoto antibodies?

I was diagnosed with hypothyroidism and Hashimoto's years ago when receiving similar blood tests. Have they checked for Hashimoto antibodies?

@intheorchard I too was diagnosed with hypothyroidism and then later with Hashimoto's. I have numerous auto immune system issues and Hashimoto's is just one of them. All of this came about when I was diagnosed with Afib and hypothyroidism can trigger Afib.

@suerte I too was Dx with hypothyroidism and later Hashimoto’s with numerous autoimmune issues etc. but my heart wants to act like I have hyperthyroidism and I was Dx with SVT last week! I often wonder what in the world is going to be next!

I’m sorry you’re suffering. I am 69 years old, a woman and have been diagnosed with Hashimoto’s and Hypothyroidism about 3 or so years ago. My TPO Antibodies were also 600 and later came to find out that I had Long Covid! Neither myself or my doctors knew what was wrong with me, but prescribed the necessary dosing to manage Hypothyroidism, which at the time was 125 mcg of Synthroid and 5 mcg of Cytomel. I immediately felt better and the dosage was eventually cut back to only 75 mcg of Synthroid, as my Long Covid was making my Hypothryodism worse. I have learned through reading and doing my own research that Hashimoto’s is controlled by diet alone and the symptoms you’re having were mine and when I got my diet under control, they went away. I have eliminated eating gluten, dairy, soy and seed oils and the body inflammation and headaches, hives, all gone. I have no taste or smell due to Long Covid, which does make this diet easier, but by no stretch easy! I miss food and the way it tastes. Try eliminating these highly inflammatory foods and it should make a difference and pretty quickly too. All the best!

did you have high TSH and HIGH T4? I am not sure my labs are fitting the description for hypo and Hashimoto's. I just got labs done again yesterday, which marks 8 months and my TSH and T4 are still high.

I definitely believe diet has a big part in relieving symptoms; I guess I would like to just figure out what is going on. I am starting to believe my thyroid problem is linked to another problem that hasn't been identified. A couple years ago I was deployed and had massive amounts of blood when going #2, they did a colonoscopy and said it was just internal hemorrhoids. still to this day I am bleeding. Military hospitals suck so I am hesitant on going back again for this issue. I just wonder of maybe something else is linked.

I am starting to believe my thyroid problem is linked to another problem that hasn't been identified. @alexandriagarcia0611 First of all, thank you for your service. Secondly, you are a military man and you’re being treated this way?! I’m not totally shocked, but the way you’re being treated is a tragedy! It may be just hemorrhoids, but you need a more thorough examination of your colon and stomach. I had 18” of my sigmoid (large) colon removed due to severe diverticulitis in April of 2022. A lengthy surgery, but no bag was required thankfully! I also since having this surgery developed Hashimoto’s and Hypothyroidism, as I never had it before the surgery. I was gaining weight by the day, literally and knew something wasn’t right. My daughter, who had a total Thyroidectomy due to thyroid cancer, asked me to have my Thyroid Labs run. To my and my doctor’s surprise, I had both Hashimoto’s and Hypothyroidism. He put me on medication immediately, Synthroid 125 mcg and Cytomel 5 mcg daily until my thyroid labs began to drop and now I’m only taking 75 mcg of Synthroid. To control Hashimoto’s, I’ve eliminated eating gluten, dairy, soy and seed oils which are highly inflammatory foods. Guess what, body inflammation and weight came off! I guess if I were you, I would talk with my Gastrointestinal doctor about the bleeding and have a full Thyroid Panel which should include: TSH, T3-free, T4-free, Reverse T-3 and TPO Antibodies. Be sure to include all of these markers for the Thyroid Labs. Some doctors don’t test TPO Antibodies, but that’s where Hashimoto’s will show up, if your numbers are above 10. Mine were 600 and I brought mine down by diet. Wishing you all the best!

Thank you, I appreciate your quick response, I am a female but yes, that's just the way the hospitals work for military. I won't say all of them are bad but that's just my experience. I am sorry to hear that, but relieved that the surgery went well! I have gotten all of those labs' multiple times, my most recent is still the same, high TSH and high T4, all of my T3 's are normal and my antibodies (AB is 600 iu/ml) and (thyroglobulin is 4,000 Ui/ml). They haven't diagnosed me yet, since I am experiencing both hyper and hypo symptoms. The endocrinologist is stumped because he's never seen someone with persistent high TSH and HIGH T4. It's not common for Hashimoto's, the beginning stages of Hashimoto's may cause high TSH and T4 but not over a span of 8months. I have my next appointment with my ENDO next week, I am nervous. I have been experiencing crazy symptoms for over a year now with nothing to help. Whatever it is is also causing my bones to heal slow. I had an Austin bunionectomy in Jan this year and my bone still isn't healed from the surgery, even my podiatrist was concerned because it should have healed about 5 weeks ago.

When I was first diagnosed, my TSH was high and T4 was, I believe, low but I can't be sure as it has been so long. They tested for Hashimoto antibodies and found them. Have you been tested for them?
