Smoldering Multiple Myeloma SMM Bloodwork numbers

I am interested in what everyone's numbers were at time of diagnosis of Smoldering Multiple Myeloma SMM? Were you Kappa or Lamba?
kappa free light chain in(mg/L):,
K/L Ratio(mg/L,):,
M-Spike(g/dL):

If Lambda
Free Lambda Lt Chains,S (mg/L):

Hi and welcome to the plasma Cell neoplasm world. Your numbers to me (while not a doctor) are MGUS since your M-Spike is .5, once it gets towards 1.4 I would make sure a BMB is done as a baseline. They will check the involvement of the monoclonal cells, the (CTC) within the bone marrow.

@dmdinapoli81 my mspike started at 1.5 (that’s at the level when we first tested and discovered it. That’s why I had the bmb which showed the %abnormal plasma cells that put me in the SMM catagory. Interestingly, my mspike has been 1.3 then 1.2, and lab last week was 1.1. I know that reduction in that number is only significant in that it is not rising. When it rises, or other symptoms develop, we then must investigate further. Nothing replaces the information gleaned from the BMB. That’s the gold standard of information.

Sorry I thought you said your M-Spike was (.5), I am super happy for you that your m-spike is going down. I have another BMB being done in July. Have you looked into the Plant based diet clinical trial? I heard that is looking into reducing numbers.

My doctor is cautiously optimistic that it's not MM, pending scan and biopsy results. The concerning factors are the rising Kappa lambda ratio, the pressure I feel in my upper left abdomen and the fact that I didn't get full PMR relief with 25mg prednisone and the pain is actually returning in the past week. I'm also severely fatigued even with taking belsomra no matter how much I sleep. My calcium is also moderately high.

@dmdinapoli81 my first mspike was 1.5 which is why I had the bmb . But mspikes do vary from draw to draw and I know that. But very pleased it is not going up!
Yes, I have researched plant based diet. I was born with a heart defect so protein is important for my heart health. I do eat a lot of plants and some meats in moderation. I’ve really cleaned up my eating, with a rare sweet as I do like that. I stopped all alcohol. Just not worth it. I’m at a good weight, I exercise but less than I used to. I just get too fatigued otherwise. And I take tumeric capsules. This might help according to my MM specialist. And I avoid stress as best I can. The rest…I can’t control.

The IMF just stated in an interview that a small portion of the population will have paraproteins that will clone themselves. with an m-spike of .13 i would not worry at all. if it gets up to 1.6 have a baseline workup done, you can also go to the iStopMM website and look at the calculator. https://istopmm.com/riskmodel/

but only if your m-spike gets up there.

I saw my hematologist oncologist yesterday and my bone marrow biopsy results are SMM with low risk, but because I'm on prednisone my numbers are somewhat understated. I'm tapering down on prednisone and hope to be at or under 5mg before my next appointment in August.
