Brinsupri (aka Brensocatib)

Posted by scoop @scoop, Aug 12, 2025

FDA approved Brensocatib today. Hooray for us. Let’s hope we have access to it as soon as possible and it gives us some measure of relief. Thank you to all the scientists and doctors and patients!

@jnmy I can't say with regard too how Brinsupri will be handled long term. I can tell you that both my insurance company and my daughter's have pushed back against preventative therapy with certain meds. In my case, the doctor convinced them to change. In my daughter's case, her company would not budge and she is paying out-of-pocket using her HSA dollars.

I'm starting on Brinsupri for my Bronchiectasis and I would like to know from others their experiences with taking it and what side effects they noticed. Also I've heard that Carbocysteine is helpful for getting mucus. Thanks. Jim D.

Hi, welcome to the group. Brinsupri has been discussed a lot here with people sharing their various experiences. The biggest takeaway is that it can take up to six months to fully work. I hope that reading through these comments might help answer some of your questions. If not, please post in one of those discussions so it’s easier to find the discussions.

Hi, I have had MAC since diagnosed in 2006. I am on oxygen now and a doctor just suggested I try Brinsupri. I was looking through the brochure, and several side effects seem concerning: areas of skin thickening, gingivitis and alopecia. I have eczema, which causes rash and skin thickening, and I can't imagine dealing with more. I also already have some alopecia. So these things make the Brinsupri concerning. Could anyone tell me how bad these side effects get so I can decide whether to start this medication? I would so appreciate your help. Thank you, Ellen

@authoreg
Hard to say. Some get side effects and others do not. It might be helpful to read through the forum under the keyword Brinsupri, which will give you first hand accounts of experiences with the drug. I've been on it (25mg) for almost 9 months. So far, I have not experienced alopecia or thickening of the skin. I'm careful to brush 2x/day and floss 1x day with a rinse and have not experienced any gum bleeding etc. It's important to read through the clinical trial data and drug leaflet and discuss with pulmonologist. If you do get side effects my understanding is that they are reversible once you stop the drug.

Even while taking Brinsupri, it’s still essential to stay consistent with airway clearance. That will never change. Airway clearance remains the foundation of treatment because it helps remove excess mucus that can make breathing more difficult. Brinsupri works by reducing inflammation.

@authoreg Ellen, do you have bronchiectasis in addition to a MAC infection? Brinsupri is not an antibiotic and so is not prescribed for MAC. How are you currently treating the MAC? Best of luck.

Thanks Toni, yes Ido have both MAC and bronchiec. I have been on many differentantibiotics. I am on oxygen24-7 so I am looking for a way beyond the nebolizer to reduce this heavy mucus. I am having great difficulties sleeping as the mucus symptomsarethe worst then. Anyway, thanks so much for helping. Ellen helping@tchd518
