Benign prostatic hyperplasia (BPH) Surgery Options

I had a Urolift procedure last February for my BPH symptoms and it was a total failure. The implants are still in position though. I am looking for a different procedure to alleviate my BPH problem and Rezum seems to be a good alternative. Can it be done considering my prior Urolift procedure. If not, what would be your recommendation.

I had BPHlaser vaporization surgery 5 months ago,developed urethral stricture within a month, hat optilume procedure for stricture, done 3 months ago.
Every so often I pass brown small ?scab and other times red about 2 inch long thread like clot.at ti.es few drops of fresh blood oozing out.
Still has frequency. No UTI cancer stones.
I am not happy with all these post surgery issues.
Any wisdom?

I also had a laser (green light) for BPH and developed a stricture which required another surgery. Instead of optilume my surgeon used the green light laser again. My initial recovery (first few weeks felt the same for both), but after repair of the structure within a few months I was much improved without frequency or urgency. I did have some occasional small bits of scab come out, but very little bleeding. It sounds like you are still (slowly) healing from the optilume. If your urologist feels your symptoms are just part of the recovery process, it may just take some time for them to get better. It never hurts to get a second opinion at a regional or national center like Mayo Clinic or Cleveland Clinic if you are not happy with your progress and think something might be wrong.

May I ask you a question regarding post Laser BPH complication stricture location in yoursituation.
My stricture was in bulbar urethra 2 cm.long.
I am 66 years old.

I'm currently part of a research trial at Cleveland Clinic called the Expander 2, and totally unhappy with the care I have received up until now. The device was implanted in mid-March. It was supposed to be a 3 month blinded procedure but I was unblinded and knew from the start that I had received the device. In the event of complications on the day of the procedure I was not given the proper instructions to contact anyone. I had complications and was only able to contact anyone due to being accidentally unblinded and having the ability to then use MyChart. I subsequently continued to have many side effects expected in less than 1% of the participants. The coordinator was supposed to be tracking this and was not. Now almost four months post procedure I have blood in my urine and they are telling me this is normal and to be expected. The twenty five page document explaining the trial and possible side effects never mentions blood in the urine beyond 2 days post procedure. Don't assume that that because the institution has a prestigious reputation they will treat you accordingly

@mikena - I'm sorry you're having such struggles. I don't think you asked any questions so all I will do is mention the obvious need to raise your concerns directly with the senior people at Cleveland Clinic. What you describe is both unexpected and out of character for them.

The census here in MA is that green light (HoLEP) is the way to go. I'm not convinced. Not too many urologists here do Rezum. Rezum seems the best post-surgery if it can be done with implants already in place (from Urolift) and causing a procedure problem. Looking for anyone who has done that.

April 4 - 8 weeks post-op
No visible signs of blood since March 31

Urine flow sprays if my bladder is not full and streams when very full.

Stopped, Cell Salts, April 1st. They increased the time to start a stream to around 30 seconds on my night time bathroom trips.
The cell salts contained antihistamines and pain relievers.
I believe the pain reliever helped reduce the trips to the bathroom and the antihistamine caused the delay in starting a stream.
Taking ibuprofen before bed is a common practice to reduce your bathroom trips, but it’s hard on your liver.

I take 400 mg Ibuprofen at night. Gets me 5-6 hours sleep before first pee.

I am a standard issue 75-year-old man with self-diagnosed BPH. I get up in the night every 1-2 hours to urinate. I have considered take one of the processes that lessen the restriction of the urethra to relieve my symptoms. In addition to the good feeling that a full night’s sleep provides, my main concern is that disruption of sleep can advance Alzheimer’s Disease. There are many articles in the popular press as well as professional journals that provide me with the motivation to act. That insurance will cover of the procedure lessens any hesitancy. Today the Rezum process seems to be the most preferable to others. Have any readers tried it? Have you found success with other procedures? Both pros and cons would be helpful to any decision I would make.
I am taking the doctor’s recommendations on the way to a permanent action. So far, he has diagnosed an Overactive Bladder and prescribed Myrbetriq (mirabegron). I have taken it for about a month and it has reduced my urgency to urinate, which is welcome, but it has had no effect on my sleep interruptions, my main concern. I fully expect his next recommendations will be the non-medical steps of Sleep Hygiene (exercise, no caffeine after 4 pm, avoid alcohol, etc.) before offering a referral to a urologist. Any of your histories and recommendations would be welcome.

I am 82 with BPH. Have and still am considering REZUM but for some reason every time (twice) I was ready to do it, something prevented it. First the urologist lost access to machine, then it was my PSA at 4.3 leading to many PC tests etc......Tamsulosin still works and Ibuprofen helps sleep at night. Now I take multiple supplements that help prostate and it actually shrunk from 87 ml to 73 ml in one year. Go figure. Lots of info and mis info out there. Tough to decide. Good luck.

I had REZUM. It didn't work. I am waiting for HoLEP. PROBABLY 6-8 weeks.

True I have tried it in the past when I was not on Coumadin which is an anticoagulant. In other words it takes me a lot longer to clot. Ibuprofen ,aspirin, things of that nature reduces my ability to clot in a reasonable amount of time .
