Wegovy then switched to a compounded formula of semaglutide

Posted by bhb30602 @bhb30602, Jun 9 12:36pm

A month ago, my PCP gave me a free sample of .25 Wegovy (month's supply) and I take the 4th jab tomorrow. Since my insurance won't cover Wegovy, he prescribed a compound formula of semaglutide with pyridoxine and glycine and increased the dose to .50mg. The pyridoxine (B6) for possible nausea and glycine to help maintain muscle. I've checked out the compound pharmacy on BeSafeRx and everything seems to check out as reputable.
Has anyone started on Wegovy then switched to a compounded formula of semaglutide and if so, what was the transition like and have you lost any weight?
Any info is appreciated.

I understand that totally. We have no kids but our girls are our family. We lost our 2 girls within months of each other..it was devastating. They are family. We waited almost 2 years to get another dog..then another so we have 2 girls again! That’s a normal grief reaction. Take your time and grieve the pups, no reason to hurry it. I can now laugh and smile and think of our girls but it took awhile..a long while. One was my heart dog, we called her.
Did your Dr give you something for nausea? Zofran helps a lot of people. I take my shot at night..10 pm maybe I slept thru it. I didn’t have nausea except a couple times when I ate too much like a whole steak…that was the last time. You will learn what you can and can’t eat anymore. I just felt like I had gorged myself at a buffet all the time. I would eat a couple bites and I couldn’t eat any more. I thought that is how these drugs worked, I thought everyone felt like that and that’s how you lost the weight. I couldn’t hardly get up and go places without being dizzy and faint from not eating. I talked my endo into reducing my dose and we have both been surprised that now I can eat with no problem and still no food noise or cravings. I get hungry once in awhile but I have been told hunger is not your enemy and it isn’t. Even if I’m hungry i don’t go crazy. The journey has been good for me. I am thankful for the compound because the starting dose pen I would have had to stop. I hope you can find a local compounding pharmacy if that’s what you choose. I was familiar with them because one of our pups had bladder cancer and the other one had kidney failure, they were small so we had to use compounded meds with them. As a nurse I was leery at first but I am very comfortable after using this since July of last year. I had no choice, my gastro said Do meds, surgery or die early. I wish you good luck! PS..I have just started into therapy after 21 years, trying to find other coping mechanisms, other than eating! Only my 3rd visit so hopefully it will help.

@3oakley2 - I'm so sorry about the loss of your dogs. Wondering how you are doing with that and also with your compounded tirzepatide (Zepbound)?

My husband, bless his little heart, recorded me snoring and stopping breathing so that’s what got me to the sleep study and a CPAP machine. But it always isn’t readily noticeable. The cardiologist was having a hard time regulating my husband’s BP and I asked him about a sleep study and yep he needed it but I had never and to this day have never noticed him stop and start breathing but it’s helped him tremendously. And sleep is when your body repairs itself. When menapausal some of us didn’t and don’t get good healing sleeps.

A sleep study that your Dr can order. Snoring is a sign. Zepbound has been approved for treatment of obstructive sleep apnea now. That doesn’t mean that your insurance has to approve it you would just have to ask your insurance. Sometimes the insurances want you to do steps A, , and C before they give a PA..that’s a question you can ask them.

Thank you for understanding, my doggies are my life. we did everything for them and yes it is uber devastating, i still find myself crying and not able to deal. i so thankful for my kittie Ellie, we rescued her last year, i now think it was a sign, she is here to help me thru this horrible loss. Both our doggies also had cancer. she had spleen and he had pancreatic.
I do have zofran, thank you for the reminder. I will take it next time i have that horrible feeling.
We bought protein shakes from costco and they have been a huge help, since I am also a vegetarian and do not drink milk we found some that are vegan and help when im in a rush.
i have been thinking about therapy, ive done it before for childhood ptsd maybe i need to revisit. good luck w therapy, please keep me posted!!! it gives me hope!!! i keep repeating time heals. its going to be a year in october for Isabella and i'm so not ready.

sooo happy for you!!! this is fantastic!!! I will have to look into finding a compound pharmacy. I am on my first week of the tirzepatide and wow with the nausea and stomach upset. I find that if I drink lots of water and eat super healthy including veggies with my protein it helps. I used to be more motivated to move, but we lost our shihtzu girl last october and it literally broke me and then in May we lost our cavapoo. So I have been dealing with a lot of grief and emotions since we don't have human children, the doggies are our family. I have not been as active since her loss and I am still super depressed but at the same time I am starting to want to feel better about myself and I think that by working on my outside my insides will catch up. + I really want this to work permanently. I think that if I tacle it from both sides it will work, keeping fingers crossed!

Do you happen to have sleep apnea? When I started last July ‘24 it was not a covered but now it is. My apnea has so improved I’m sure it wouldn’t cover mine now but I am going to ask next week when I see pulmonologist. Good luck!

Hi Lisa,
Thank you for your response, after using wegoy i also gained the weight back. its very frustrating. My endo agreed i should try tirzepatide, but it is sooo expensive!! for some reason my insurance does not cover it now, but they did cover wegovy a couple of years ago. my plan is to try it for a couple of months and also excersise every day. I did find a zumba class on youtube i like ... we will see!!! i think that if i motivate my brain this time it will be easier.

I have been on compounded tirzepatide-zepbound for 14 months and lost over 100#. I have 20-30# yet to go. I was unable to tolerate the starting dose, couldn’t hardly eat. Talked the endo into decreasing my dose instead of increasing it. I’ve been doing 1.5mg every for a whole year. I use a compound pharmacy where I live. My endocrinologist gives me a compound. I can eat now but still have no food noise or craving. I know I am not typical but I work very hard at dieting. I count my protein and calories daily. Making sure I get fiber and enough fluids. I am moving more using resistance bands for strength. I’m 68 years old. Off many meds. Blood work is normal. Yay!!
One thing I have noticed in my GLP-1 groups that all telehealth companies are not equal. I would suggest if you don’t feel your compound is working to find a reputable compounding pharmacy and try another. I may end up with telehealth when I get on maintenance because I have a concierge endo because I had to find one to take care of my post cancer and that would prescribe me compounds. If you can find a compounding pharmacy in your city or near you to get your meds that is affordable it’s easier and they are easier to deal with if you have questions. Also the tirzepatide works two ways and has less side effects than the semaglutide. I sure wish you well. It’s a job. People don’t understand that the weight doesn’t fall off and you have to work at it! Sounds like you know what you need to do! Good luck!!

Welcome to Mayo Clinic Connect, @loresull.

I personally was prescribed semaglutide (Wegovy) and lost 53 pounds. However, I had really challenging gastrointestinal side effects and finally asked my doctor about switching to tirzepatide (Zepbound). My husband, meanwhile, started the semaglutide (Wegovy) as well. He had no GI side effects and lost around 30 pounds on it before hitting his insurance cap for coverage of weight loss shots. He's since gained back 20 pounds.

In retrospect, switching weight loss shots was a mistake for me, mostly due to running into an insurance cap for weight loss medications and losing time. I hit this cap right about the time I got to the highest tirzepatide dose, as I had to start at the bottom dose and work up to the highest one. The tirzepatide did not suppress my appetite like the semaglutide did, which was crystal clear. I also had not lost more than 1 pound on it before I got to the insurance cap. This seemed odd, as I'd understood tirzepatide to be more effective than semaglutide. But it didn't turn out that way in my case.

In order to test to see if the semaglutide was causing my GI distress, I had to be off all weight loss shots for a month. Then, as I mentioned, I had to start all over with the tirzepatide. All this led to my gaining back 24 pounds. Super frustrating.

All this is to say that one's response to these weight loss shots appears to be highly individual. I would consult with your doctor and look at the best plan for you with any medical insurance caps you may have for weight loss shots very much in mind.

What does your doctor recommend you do at this point, loresull?
