Worried about results of Troponin test

Posted by bigheart12345 @bigheart12345, Nov 1 12:25pm

Hi, had HCM asymptomatic for 20 years (helped by bisoprolol). Just had a great set of tests with a new cardiologist who said everything looked positive as there hadn't been any change in 20 years and my Mum who is alive at 92 has the condition. Had my first troponin test (38) and BNP test (+900) both abnormal, the BNP wasn't a worry for my cardiologist and he stated at my consultation that he expected the Troponin to be normal, which it isn't. Waiting to hear from him on this. Unfortunately whilst I have lots of historic values for my HCM, nothing historic for these tests. Just wondering whether this is normal for other HCM sufferers who have some history on whether there have been changes in these.

Elevated Troponin I, CHF Peptide, pro BNP

Just looked at my lab results and all of the panel is good except elevated Troponin I, CHF Peptid, pro BNP and am scared! I am on verapramil and atorvastatin and struggle on the verapramil with seeming to "push" myself through some activity. Metropolol made me feel better, other than angina, and now wonder if either are necessary? Could this spike be a result of the COVID vaccine? just asking? Thanks

Yes! I just had my troponin test, pro BNP and my numbers were similar to yours. Caused some anxiety for me today. Been diagnosed with HCM for 8 yrs and feel that it is always changing. I take atorvastatin and verapamil, but was on metropolol. Felt better on metropolol and am waiting on if I could switch back. Was briefly on verapamil and metropolol together and it was too much. Thanks for the support that this group allows!

Troponin and BNP are measures of different proteins present in our blood that point to "heart failure." Elevated numbers, according to my cardiologist, are usually found in those of us with HCM as well as in patients with other heart conditions. In my case, my high numbers are monitored for changes, all is well if there are no significant changes. For example, I had Lyme during the early part of the summer. My BNP rose from around 400 to 1000+ which was very concerning. Doctor said to retest after a week on the antibiotic, it nosedived to in the 300's. My troponin was 14.8 at the same time. Reading about what these test numbers mean and discussing your particular situation can put one's mind at ease along with feeling that you are participating in developing your care plan.

@walkinggirl my cardiologist was reassuring wrt the numbers due to the absence of symptoms and the stability of all the other parameters. I am going to have the troponin prior to the echo next time as I think I will be more relaxed and it might be more relative to my normal numbers. I just need to keep track of it now.

@bbuck I was really anxious about mine too but have been reassured by my cardio and will look forward to a yearly test now. I just take bisoprolol and I tolerate it really well. I have tried to reduce it but I need it, it really makes me feel good. Hopefully I can keep on a relatively low dose.

I had a heart attack 8 years ago. Newly diagnosed with hocm and my Troponin is 28 which is low abnormal. They said they are looking for trends.

@rebaheizer hope you are doing ok after your diagnosis, just goes to show with these blood tests that they can be quite confusing and trends are the key.

Hello @bbuck,
Having access to ones lab results can be a blessing...or a curse...and both at the same time.
Unless you are medically trained to interpret these findings, it may cause confusion or result in more questions than answers. And you even mention you are now "scared!"
There is so much information out there about HCM and elevated Troponin/Pro BNP. There is definitely a link to these abnormal findings.
I was on beta blockers, didn't tolerate them.
Put on calcium channel blocker. Didn't tolerate them.
Put back on beta blockers.
It was a Merry-go-round of pharmaceuticals. And they never worked for me. Like you, I had to push myself to do anything. I was like a limp rag.

My advice to you is to be sure to go over these findings with your cardiologist. Have them address your concerns.
As long as they are up-to-date on all things HCM and HOCM they should be able to reassure you that these labs my not be normal, but "normal" for HCM. If there is any indication of HCM progression based on these results, they can go over that with you too.
HCM is a sneaky disease.
It can take years to progress. Or not progress at all. Or all of a sudden you need open heart surgery! It's good you are paying attention to yourself. You need to be your own best advocate in this HCM journey.

I am not an expert in anything, so there is no way I could answer your question regarding the Covid vaccine. There is a lot we just don't know yet...so not even going to venture out to attempt a guess!
That's my best answer!

When do you see your doctor again? Have they received the lab results too?

@bbuck, I moved your post to this exisiting discussion where you've connected with @bigheart12345 and others regarding changes in test levels.

As @walkinggirl @rebaheizer and @karukgirl point out, there are several factors that can cause test results like Ttroponin I, CHF peptide, pro BNP to be elevated and that your doctor will monitor you and watch for trends over time. Be sure to let him know that you received your vaccination close to the time your lab testing was done (assuming they both happened around the same time).

I echo @karukgirl's question: When do you see your doctor to review the results?

@karukgirl I was just in to talk to him, but had labs AFTER the visit. Seems backwards. I am calling his office tomorrow and hoping I get calm from the scary portal results. Thanks for the support:)
