Significant bleeding from penis. No pain.

Warning - This is pretty gross stuff.

I woke up this morning to somewhat bloody underwear. A few hours later, my underwear was pretty soaked in blood. When I investigated my underwear further, I saw a very small, membranous looking string of some sort. Very similar to when a woman menstruates.

I'm not experiencing any pain. This is not occurring during urination or ejaculation. I have not experienced an injury of any sort. No rough sex or any trauma. I did not feel or notice the bleeding taking place.

Something similar actually happened to me about 11 years ago when I was 24 (before I had health insurance - a very unhelpful doctor checked my colon, which was fine. no other tests).

I'll talk with my doctor tomorrow, but I'm curious if the community has any ideas. Most of what I see online does not really fit these specific symptoms (usually involves urination, trauma, pain, etc.).

Any ideas?

I have had this ongoing problem on and off for almost three years now. It's completely painless every time. Sometimes it's copious amounts of blood, others just a drop. Most often with clots. It stopped for almost a year in 2023 but now it's happening at least once a week and lately more than once a day.

I live in Los Angeles and have access to great health professionals. I am on my fifth urologist now. I've seen doctors at UCLA, Cedars-Sinai and at private clinics. I've had three cystoscopies, multiple CTs, MRIs, and even a fulguration to attempt to find the source and burn it in my urethra. I've also seen a nephrologist and a hematologist and no one has been able to figure it out. I am in my late 50s, healthy by any standard. No cancer, no STIs, no major health concerns or issues. My prostate is normal, I do no contact sports that would cause injury, and I have never had kidney stones. Just persistent bleeding from my dick.

It is impacting every aspect of my life now. It is oddly comforting to know that many others have been through this but I don't see anyone on this thread who has had a resolution. If anyone has, PLEASE share your experience here and what worked.

In my very similar situation with this problem, it was my very vascular enlarged prostate that caused the bleeding. Prostate Artery embolization fixed the problem. You say normal prostate so I assume not enlarged?

I am having the same problem and I don’t know what to think of it. It’s very scary to wake up to. Waiting to see my urologist.

What did urologist say? I am having the same problem. I am going to ER now.

"Bleeding from the penis or having bloody urine is always a reason to see a urologist” - my Urologist.
There are a lot of things to check.
Kidney or bladder stoned can irritate tissues and cause bleeding.
Vascular tear to prostate.
Some cancers (unlikely).
Weird that you just woke up with it. That probably means that the bleeding is coming from past the bladder sphincter. They might want to do a urethroscopy to take a look. (Dont worry, it is not as bad as it sounds)

I just retturned home after 10 hours spent at ER. My bloodwork was done, I had a CT scan, urine sample etc. Nothing significant was found but enlarged prostate (which I knew of). Blooder, kidneys OK. So they found nothing. Today in the morning same thing blood spill from penis in the morning and some blood in urine over the day. I was referred to a urologist for further examination. I wish @hopeimok could answer since he posted his case in 2021.

i have. i found a lump of skin at the end of where i was circumcised a few years ago. me and my Urologist believe that's where it's coming from. i"ll see a Dermatologist at the end of this month.

I hate when people write what happened to them, ask questions and then go to the doctor or udergo a procedure without any follow up here. We all can benefit from it.

Sadly, yes. I met with an interventional radiologist, at the recommendation of my urologist and hematologist, in the hope that he can help. Current theory is/was that there's an AVM (arteriovenous malformation) causing the bleeding but none have been seen on the CTs I've had done. I'm going to have a CTA (CT angiogram) to see if they can spot the problem. Then hopefully fix it. The radiologist didn't seem optimistic though. I hope he's wrong.
