Significant bleeding from penis. No pain.

Posted by hopeimok @hopeimok, Jul 18, 2021

Warning - This is pretty gross stuff.

I woke up this morning to somewhat bloody underwear. A few hours later, my underwear was pretty soaked in blood. When I investigated my underwear further, I saw a very small, membranous looking string of some sort. Very similar to when a woman menstruates.

I'm not experiencing any pain. This is not occurring during urination or ejaculation. I have not experienced an injury of any sort. No rough sex or any trauma. I did not feel or notice the bleeding taking place.

Something similar actually happened to me about 11 years ago when I was 24 (before I had health insurance - a very unhelpful doctor checked my colon, which was fine. no other tests).

I'll talk with my doctor tomorrow, but I'm curious if the community has any ideas. Most of what I see online does not really fit these specific symptoms (usually involves urination, trauma, pain, etc.).

Any ideas?

Hello @hopeimok - Welcome to Connect. Waking up to blood is kind of scary and especially in your situation. I had a similar bleeding situation that I noticed during the night but the bleeding was from my ankle area and had coagulated multiple times during the night and opened up again when I went to the bathroom which is when I noticed blood spurting out of my ankle.

You probably already found these possible causes but I thought it might help if not -- Bleeding from penis: Causes and when to see a doctor: https://www.medicalnewstoday.com/articles/bleeding-from-penis

Mine occurred because I rubbed my feet together and I had a untrimmed toenail that nicked a blood vessel near the skin surface. It's good that you are seeing a doctor tomorrow and I hope you are able to provide an update as to what was found. You mentioned no pain or injury. Did you see any area of bruising which could indicate blood beneath the skin?

Hello @hopeimok

Well, welcome to world of What's this now? Since you are only 35 an enlarged vascular prostate may not be the problem, but I have had same issue caused by said prostate. No pain and with the tiny stringy clots. It happened twice so second time my Uro wanted to do the dreaded cystoscope procedure to rule out bladder cancer, which it did. It doesn't happen anymore because I have my enlarged prostate under control. By all means you should be consulting with a Urologist. Good luck, hopefully just a popped capillary somewhere.

You definitely need to see your PCP ASAP. You didn't say if there were any clots or just red blood. Was any of it dried or fresh, color bright red or dark? These are the questions your doc will ask you. Could be a component from your diet from prior meal, your bladder, your urethrea, and too many options to choose from but it's not usual. Seeing a urologist is in your future. Good luck my friend.

@hopeimok I didn't see a post subsequent to your doctor's visit. I'm experiencing the exact same issue today. The bleeding is subsiding but very troubling. I'm suspecting that it's a damaged blood vessel but not sure what to do from here. Thanks!

@naperjohn, make an appointment with your Uro to rule out any serious causes. I think I read in my research that either it’s 10 or 20% of men with enlarged prostate experience some occasional bleeding from a highly vascular prostate. One of the culprits is having a difficult bowel movement within the last 24 hours. That was my issue and once I fixed that the problem has not returned. Going to the Urologist gave me peace of mind that it wasn’t something more serious. He’ll do a cystoscope so you’ll need to bear a few uncomfortable minutes. When I left the office I had a big smile knowing from the immediate results that it wasn’t bladder cancer.

Same story....difficult bowel movement, and then some blood out of penis. Once I started the stool softener, and changed diet a bit, all cleared up.
