Recovery expectations 6 weeks after HoLEP prostate surgery.

Posted by 2longlife @2longlife, Jul 14, 2024

I had the HolEP procedure six weeks ago. All is good except that I have a dull pain in the perineum area when I urinate. Is this because of the long healing time for the urethra? Is this normal? My follow up appointment with my urologist is six weeks away.

Contemplating having a turp procedure can anyone who has had that surgery care to share their experience as to recovery period etc. I know everyone is different in how they react to this procedure and how their recovery progresses, but any insights shared would be appreciated.

Does anyone have experience with a Cunningham clamp. My Dr. suggested this for the incontinence that does not want to go away so far. It literally clamps on your penis to stop the leakage of urine. It looks more like a torture device but if it helps to be able to do more things without changing guards every two hours I might try it. I did Physical therapy and was told I have good strength and control over my kegel muscles and yet the leakage continues five months after surgery. Going anywhere or doing anything for any length of time is a struggle.

@rwinpa
I got the clamp and is not working for me no matter how tight I put on, as the day goes by I may become flaccid and it gets lose and i start dripping .

I am thinking that I will stay on Flowmax! This HoLep doesn't sound good, I had enough problems with Aquablation that only helped 7 months and prior to that a Urolift that didn't help at all. Seen doctor yesterday and asked why Aquablation only lasted 7 months and not 5 years or possible 10 years like promised, all he said was guess they didn't take out enough tissue, would I like another procedure. Said NO, I getting by on Flowmax. Interesting that you folks are having this leakage, I was told there only a 1 percent chance of that happening.

@diverjer I was told the 1 percent story also, but that is not accurate. The first several weeks to months is way higher than 1 percent. They also told me I was "YOUNG" and 'healthy" and would be "FINE". 14 months later and I am still leaking and they are suggesting an "aus" surgery now. The doctor who did the Holep quickly ditched me after my 6 month check up. Basicly said sorry for your luck. It was done at Penn State Hershey in PA. Make sure you find a doctor with alot of experience/practice with the Holep. Once they damage your sphincter there is no going back.

I think it's normal. If you search for "HoLep log what was your pre and post-op experience". This is where I starred and posted my holep experience

To this day 5 months later, I still get shooting pains like a shock every once in awhile from my rectum to the tip of my penis. I just had my check up last week. He said everything look good. Did not express any concern about the shooting pains. Also whenever I go to number two. If I push too hard it's like I'm missing something for my muscle to push against. I have actually pulled a muscle down there..

@nabulldog I had same surgery 7 weeks ago and experiencing the same shooting pain esp toward the end of emptying.

Has you’re improved since 2024?

@rob113 yes, it took maybe a couple more weeks before I noticed the feaquency decreasing. The only issue I have is retrograde ejaculation, which was expected. Small amounts of s
Semen comes out of the end Maybe 25% of the time .

@nabulldog thanks for your reply. The retrograde is understandable and also expected here. It’s the urgency, frequency and the on and off sharp shooting pain after urination that I am still dealing with at week 7. Strange how it comes and goes. I may have been doing too much yard work!

@rob113 if I remember correctly, it basically was muscle spasms or cramps I was. Feeling. As you heal they will go away. In the meantime it's hard to get used to the random shocks you feel. Try not to push hard to urinate. In my case I had to push pretty hard at first in order to get anything to come out. As I healed that became less and less. I believe at one point I pushed a little too hard and it felt like I might have pulled something. That didn't help my situation at all. In time it will get much better

@nabulldog thanks again for your replies and support. Although I wouldn’t wish this on anyone, I’m glad to see I’m not alone in these challenges
