Did your surgeon discuss impact of prostate removal on orgasm?

Someone mentioned that not enough guys experiencing positive post-RP results comment on this portal. Here's my journey. 62 yo, I had RP almost 5 months ago.
Incontinence was significant the first two months, to the point that my daily activities were very much effected by embarrassment, shame and fear of public humiliation. It began to improve in the third month and the progress was steady to present. For most days, I'm mostly dry, with occasional stress-leakage. All of the above is not relevant if I consume alcohol, in which case it's nearly a free-flow situation. I still wear some kind of leak protection, and more often that's by using some of the boxer-briefs that have an absorbing panel in front of the penis. They work surprisingly well. I think paranoia keeps me from NOT using the protection, because during a recent vacation I found that just wearing board shorts during water activities was fine.
Erectile dysfunction. Initially, there was very little erectile response. My surgical team advised to not even try for one the first six weeks, during which I was on 5mg of daily Cialis. After that period, I had some slight stiffening, and sometimes enough for penetration, but it was inconsistent. Full doses of Cialas or Viagra helped.
For the first three months post-op (and because of both of these condition) I believe I was suffering from significant depression as a result of grieving and fear.
Around the fourth month I read somewhere that besides the obvious physical implications of the surgery there is often a psychological factor that contributes to the ED. From that point on I've made a couple adjustments that seem to help. First, I make it more of a priority to masturbate often, while simultaneously keeping my expectations low and a focus on enjoying the physical sensation. Additionally, my wife and I are more diligent around including more arousing activities pre-sex (flirting, talking, erotic material, etc), and again we keep the expectations low regarding an erection. I acknowledge that some of the above might be controversial, but we're pretty open to any idea that might improve the recovery.
From the fourth month until present, the incontinence, the erections and the emotional/psychological has improved. I set aside some time recently, without the ED meds and attempted to achieve an erection. It was mostly successful, but it required more than usual stimulation and patience.
Orgasms are slowly returning. I find that there is very much a mental component. If, during sex I can maintain a strong level of mental excitement and stay "in the moment" then the climax is very enjoyable. On the hand, if I become anxious about performance or my thoughts stray to places that aren't highly aroused then the orgasm passes and is barely noticeable.

9 Reactions@daveandoneita I feel your pain, the few times I’ve been lucky enough to for a minor erection for penetration I’m not feeling anything senual and no chance for an orgasm. Do you feel any sensation or like me it feels very mechanical as if I had a wooden penis , this only frustrates me more so I rarely try lately as much as I miss it and occasionally get very depressed
@tdabbotts Totally agree with the mental aspect of recovery. I’m 60 years old and 6 months out from nerve sparing RARP. I resumed sexual activity at 6 weeks, but it wasn’t until I got past my fixed mindset that ejaculation was the goal of masturbation and intercourse that I was able to enjoy both.
Also at 6 weeks, my urologist asked if I was waking up with erections. I wasn’t, so he prescribed daily Cialis 5mg (in addition to Viagra as needed) and said to “coax it if necessary for daily penile rehab.” Between that and changing my mindset, I’ve been experiencing very intense and pleasurable orgasms with lots of fluid from my Cowper’s glands.

sorry for your problems I've been this way since middle of 2000s. I started with the trimix, couple month ago, just for my woman. it works for her, not me. erections are good and last for a while. still working on dosage. everything you read, is all over the board. but I'll keep trying. just some kind of a orgasm would help for me. I'll get off now, just keep tying!!

4 Reactions@robertcukor sad but glad to read that I’m not alone in that as a result of the RP there is a loss of some of the length. Coming up to my 2 year anniversary of the surgery. To the comments over the Dr. mentioning the possible side effects. He repeatedly said that I would not be having any more children. But having had a vasectomy in my early 30’s that was the last of my concern. Did want to still continue having erections but except for Trimix it’s the only way. Daily Cialis for several months after surgery did nothing. Lucky are those whose nerves were spared and erections came back on its own.
@phayes you are fortunate to have had a nerve sparing surgery that allows you to get an erection simply by using ED pills. As well have output from the cowper’s glan at the end of intercourse. As that is not my personal post surgery outcome and most of the men I’ve spoken with post RP surgery
@rlpostrp my Dr. timeline to see if there was going to be a return to being able to have erections past RP surgery was 1 year. Now nearly 2 years since getting cut. The only way to get any erection that ends in a satisfactory orgasm is with Trimix injections. As I read others reports same as my own experience the results vary with the dosage and the supplying pharmacy and vendor as along the way with the first pharmacy whose mix provided a good erection their source changed and results were varied and painful post sexual climax. Now have a new pharmacy that at first their mix provided a good result, although recently months into new vials my results are not as rewarding. My erections are short lived about 30 minutes. I’ve never had a 4-5 hr erection as claimed by others. The best has been perhaps 1 hr. So wish you all the best in your new post surgery journey.

Unfortunately even with nerve sparing surgery the erectile function often does not come back if you are over 60 years old

@peterj116 Not sure I can be much help. My urologist is at Mayo. He had his tech show me how to do the injection and I was told to start at 5 units and experiment up to 40 units for the right length of time for the results. Less than 2 hrs. He ordered through a compounding pharmacy in my area. (Strive Pharmacy) and said he ordered medium strength. (?) He also gave me the little bit of the bottle left from the office visit. I am not sure how much in ml is the 5 units? The small bottle does not say on the side how much is in it. It does say 30ml of one drug - 10ml of another - 1ml of the last. Strive charged me $95 for the bottle. The 5 unit injection lasts me about 3 hours. I have not experimented with more or less. The day I picked up the medication I drew about 10 syringes from the Doctors bottle and 10 from the new bottle and put in the freezer. The drug says it has a shelf life in the refrigerator. Ten syringes has barely used any of the bottle. I would say I could get another 15-20 syringes out of this bottle. I would say not bad return on investment. At least 20 party nights for $95. It costs me $10 for a crown and soda water.
