Memory Care Check List (subjective)

This is my subjective checklist regarding how to get my LO into memory care. There was so much to do before my LO could be placed into memory care and so much I don't/didn't know.

My Memory Care Short Form (subjective) Checklist--what to do so my Loved One (LO) could enter Memory Care

1) Contact Hospice of the Valley, Dementia Care
2) Get Social Worker assigned
3) Get Hospice of the Valley Dementia Care Dr to Review Medical Records
4) Make list of Memory Care facilities, visit on off hours, ask questions of staff. (Have a family member do this for me)
5) Decide on Memory Care Facility, get pricing, ask questions
6) Make appointment with Loved One’s General Practitioner (GP), give heads up on Dr. Orders
7) Get chest x-ray (if needed) and TB test (local pharmacy)—TB est is in two parts,
8) Memory Care Facility will send nurse to evaluate to home, help create a “story” with that person as to why Loved One will be taken to Memory Care.
9) Have GP FAX over Drs. Orders to Memory Care. Memory Care will not take LO with this
10) Meet with Memory Care “Family Advisor,” fill out lots of forms. give non-refundable “community fee” deposit (make a copy of check), decide on financial move in date (1st month’s rent in addition to community fee) as well as physical move in furniture, set physical move in date.
11) Buy furniture and everything needs for a new “apartment”—include personal items like shampoo, toilet paper etc. Coordinate with Family Advisor. Get help with move.
12) Have extended family member bring Loved Ones to Memory Care
