EPI meds expense

Posted by grannydee @grannydee, Dec 12, 2021

On Medicare, Medicare supplement and Cigna scrip insurance. My meds are NOT covered. I appealed and my appeal was denied. The “discount card” is not for Medicare patients. I cannot afford my medicine as it costs more than I even receive from SS. I don’t know what else to do. I was misdiagnosed for 30+ years and now that we have finally found a med that works I cannot afford it. I don’t know what else to do. Maintaining weight is not working, currently at 106. with osteoporosis and anemia due to the long term misdiagnosis. I am willing to try just about anything at this point.
I realize I’m not the only one but I live in a small town and just found this group. Any suggestions or help would be appreciated. I’m embarrassed that I cannot afford my medicine and while I know that there are other people my age experiencing the same issues, I just never imagined that it would be me too.

I don’t believe registered dietitians are able to clinically diagnose. They have a bachelors degree and dietitian license but limited in what they can do compared to a medical doctor.

Did your gastroenterologist do bloodwork, ultrasound, MRI, endoscopy and colonoscopy? Have they ruled out pancreatitis and any issues with liver or post gallbladder removal? Have they ruled out celiac disease?

Are you seeing an endocrinologist for Hashimoto’s and are you being treated for hypothyroidism? Are you working with a rheumatologist to see if you have any other autoimmune diseases contributing to your symptoms?

Hello!
Thanks for your reply. Yes, I’ve been seeing a GI over the course of 12 years. Everything they’ve test me for came back “normal”. I’ve had 3 colonoscopies, an endoscopy, blood work, fecal tests, etc. I don’t have chrones, celiac, lactose intolerance, colitis, etc. the diagnosis was always just IBS.

I’ve worked with an endocrinologist since 2014 when my diagnosis for hypothyroidism changed to hashimotos. I’ve never seen a rheumatologist, but my GP diagnosed me with rayneauds. Nothing official about my liver or pancreas, except that the enzymes are the only thing over the course of 32 years that have helped my symptoms improve, suggesting that I don’t have enough enzyme production on my own.

The prescription enzyme I take is 368.00 a month copay. Without insurance it is over 2000.00 a month! I’m looking for an affordable over the counter pancreatic enzyme with comparable lipase dosage of 40,000 units. I would appreciate any suggestions, as the diarrhea caused by this condition is intolerable.

The only over the counter one I’ve tried is the one I mentioned which has much less USP than prescription.

Hi everyone!
After 32 years I was finally diagnosed by a Registered Dietitian (even though my GI tested me in 2021 and 2023 through fecal and blood tests, both yielded “normal” results) with Exocrine Pancreatic Insufficiency (EPI). I’ve started Now Super Enzymes and felt relief the same day! My bloating, gas and cramping drastically reduced. After 2 weeks, I’m starting to see bloating increase and I’m feeling discouraged! My GI prescribed Zenpep and I’m just waiting on my RD to agree to switch to Zenpep. Has anyone else delt with extreme bloating and have success with Zenpep? My RD is confident that if her hypothesis is correct and I do have EPI, I can live a symptom free life.
I also have hashimotos, rayneauds, and gallbladder removed.
Thanks!

Hi, @andinatalie, and welcome to Mayo Clinic Connect. You will note I moved your post here so that you can connect with others such as @cmhgiddy24 @creondave @psnead @marylandbob @cc8 and others who have been talking about EPI (Exocrine Pancreatic Insufficiency):

- Does anyone have EPI (Exocrine Pancreatic Insufficiency) https://connect.mayoclinic.org/discussion/does-anyone-have-epi-exocrine-pancreatic-insufficiency/

When will you start the new medication? How is the bloating today?

I had a EUS (esophagus sonogram with endoscope) to confirm my EPI/pancreatitis and rule out cancer. I agree with the previous comment regarding finding a new gastrointestinal Dr. I started taking ZenPep but they were too big for me to swallow so we switched me to Creon.
Much better. Adjusting the dosage is very common so try not to get discouraged. Your dietician can help you with the vitamins you will need. I had the same results on my fecal. Please request a sonogram as an endoscope alone isn’t enough. I have cysts and atrophy of my pancreas so I need to be proactive. I hope this helps you!

Is anyone else struggling with friends and family regarding changes to your diet? Many of my friends are 'Foodies' and it is difficult to get together because they are always focused on food and drink. This for me has been one of the most difficult issues with my EPI diagnosis.

@robincat99 It was tough when I was first diagnosed but I try to tell folks to just eat what they normally would and I can either bring something special or usually find something I can tolerate. Sometimes I'll eat before I go and just make it about the fellowship. I know this is tough. I didn't even know about EPI until I was diagnosed. Sending you love!!!

I am fortunate that I've met my deductible ($10K) this year and my Creon is currently at $0 but that will probably change after the first of the year. It is RIDICULOUS how expensive life saving medication is. And this IS life saving medication. I am so sorry you have to deal with this.

There are some good over the counter options that seem to work well (AI recommended this one to me that I took before I filled the Creon: https://amzn.to/46TizgQ) - The Vital Nutrients blend is pretty close to Creon as far as covering a few different enzymes but it's not time released. The downside is you have to take them while you eat. So one at the first bite, one during the meal and one near the end of the meal. The Creon is time-released. I hope this helps. Sending you good thoughts!!!

@1realjeannie Have you tried Viral Nutrients for an extended period of time? If so, how was it compared to Creon?

Along with an increase in Creon, I have been put on Colestipol 1M/ 2 tablets twice daily. THIS seems to be helping immensely. It's been suggested that I have EPI with BAM. I have had 12 days straight with no diarrhea. However it is still too early to determine if this is the magic combination. Fingers crossed. I am still having significant discomfort in the right upper quadrant (under rib cage) that radiates into back. Will keep everyone in this forum posted.

@darb
I have just been prescribed Creon and also on Colestipol
I am struggling with the added Creon and how much to take - can you share your dosage? I’m 66 yo & weight 195 (nearly impossible for me to loose weight especially belly)
Greatly appreciate your response

I had my gallbladder removed a long time ago. Bile acid malabsorption, seems to describe a lot of what I am going through. I have doubled my dose of Creon as of yesterday. This morning has been awful. Watery diarrhea, green and metallic in smell. Continued discomfort on right side below rib cage. I am beyond frustrated. This is so not living. Does anyone know if Cleveland Clinic has a digestive department?
