Doses for the Big 3 meds for MAC
I have had MAC for about 3 or 4 years. I’ve been in the “watch” stage with no meds. I also have Wegeners Disease which is under control but the meds bring down my immune system. I’ve had pneumonia for the last couple of months so my ID doctor is starting me on the Big 3 this week. I’m curious about the dosage that others are taking. Here’s what I have been given:
Azithromycin- 500mg
Ethambutol HCL - 1600 mg
Rifampin - 600mg
All 3 x week starting with the Azithromycin 1st & gradually adding the others.
Does anyone think the ethambutol a high dosage? Im 5’4 & weigh 145lbs.

@capletcher - Thank you for your reply. I'm wondering if your response may have been for someone else. For my particular infection (mycobacterium avium complex), Azithromycin, a macrolide, is recommended along with two other drugs, three times per week. I was originally on that plus Rifampin, plus Ethambutol. I'm off the Ethambutol (been off of it for five months, my symptoms have been gone for 7 months, and I need my vision, plus the fatigue from Ethambutol was interfering with my activities). Now I'm on Azithromycin and Rifampin until September. We'll have to see what happens and deal with any changes if needed. For now, it's going well!

After one year my dosages were increased to daily, rather than three times per week (increasing nodules). I take Azithromycin 250 MG, Ethambutol 800 mg, and Rifampin 450 MG every day. I will begin using Arikayce 590 MG in one week. I also take Levothyroxine 50 MG for Hypothyroidism. I have been very fatigued and weak since being diagnosed in January 2025. It feels like untreated hypothyroidism but is actually the lung infection causing the inability to exercise, heart palpitations and exhaustion. My weight was 88 lbs. and is now 94 lbs.
@ronneee please check in with your doctors ASAP regarding the weight loss. That kind of weight loss is unsustainable. Maybe you need a change in drug regimen. There is also a dietitian, Michelle MacDonald, who has a lot of expertise with NTM patients and low weight, I am sure she would have ideas for you. She use to be with NJH but now consults privately.

1 Reaction@iart Curious: Is that "Azithromycin 250 MG, Ethambutol 800 mg, and Rifampin 450 MG" once or twice a day? Because I'm on Clarithromycin 500 Mg and Rifampin 300 Mg twice a day.

Thank you for the suggestions. I have just been referred to Jewish National Health by my new pulmonologist. He also suggested the addition of between-meal snacks as well as a bedtime one. I also just added protein drinks.

@rc4mc I am surprised you are off ethambutol because it is the accompanying medication with azithro and rifabutin. When rifabutin and ethambutol were stopped for me because of very elevated liver enzymes and clofazimine was added plus four months of amikacin IV, national jewish doctor added ethambutol to that regimen. I am sure ethambutol is part of the regimen so ask your doctor. Unless there is a reason it was stopped of course.
Thank you, @lilianna - yes it was stopped because I couldn't tolerate the side effects and was especially concerned about potential visual issues, given my husband and one daughter have a hereditary eye disease and don't drive, I have a full-time job that uses my eyes extensively and I'm also in school full time. My CT scan showed significant mucus in the right middle lobe, not much else. MAC symptom (cough & throat clearing) had been completely gone for months and I haven't been sick much at all in life so far (I've rarely ever needed antibiotics except when I was young and used to get strep throat - about 50 years ago or so). My doctor has stopped ethambutol with other patients who don't have other concerning indicators and it has worked out. I'll mention this when I see him again in two weeks and will see what he says.
@lilianna Glad you posted about the need for Ethambutol along with Azithromycin.
My understanding with Dr. McShane and nurse Tracy is that the Azithromycin must have the Ethambutol with it.
My last understanding with Dr. McShane was that they aren't prescribing the Rifampin? I think they felt it was causing too much trouble vs good.????
Something to look into for all of us with the NJH doctors.???
