Can HCM cause chest pain?

Can HCM cause chest pain even if you do not have coronary artery disease? Does nitroglycerin relieve it?

HCM Obstructive with cardiac arrest and icd. Definitely have chest pain often especially at night from the thickening of the septum and difficulty in the heart pumping. They don’t suggest nitro which I’ve had at ER and it only gave me a massive headache with no pain relief. I take a baby aspirin or two for the pain and sometimes it helps. We’re all different so I hope you find what you can live with symptom wise. Take care

Hi, i have significant Apical to Mid-Cavity or Mid Left Ventricular Obstruction. GTN Spray & other Nitrates worsened my Chest Pains. I struggle to manage my persistence chest pain episodes but have been informed I need to learn to live with this symptom.

Hi, yes unfortunately Chest pains can be in absence of Coronary Heart Disease (CHD) especially in HCM patients
I have had x4 negative Invasive Coronary Angiograms to prove this.
I have been Advised that HCM can definitely cause varying degrees of chest pain & is a well recognised symptom of HCM.

In my experience, GTN (glyceryl trinitrate) Pump Spray & other Nitrates have worsened my Chest Pains. These are usually advised against (contraindicated) in HCM due to heart muscle thickening (Hypertrophy).

I have obstructive OHCM and when I had chest pain too. Since I started taking 5 mpg Camzyos there is no chest pain anymore. Nitroglycerin will be bad for me said my Cardiologist due to my OHCM

Hi, its good to hear that u have benefited fr this medication, Mavacamten (Brand Name Camyzos) ie ur chest pains have improved.

Hi, just wondering whether u feel if ur quality of life has improved since Surgical Septal Myectomy & ICD Implant? What symptoms do u still experience?

Hi superhal, YES! YES! and YES! I was going along just fine even though my echoes over time showed a growing thickness of the ventricle wall. Then, I had a syncope 2/22. From that time on I was experiencing much shortness of breath, but no pain. Surgery was 7/22. ICD installed late 11/22 after months of dizziness with vertigo and two syncopes. The heart monitor I wore for a month finally helped my cardiologist and associates figure out that I was experiencing arrhythmias, a new thing for me. Let's face it, the heart underwent quite a bit of trauma during surgery. Since early 2023 I have been back to what I was doing before 2/22: biking, walking (did a 10K today with my walking club), gym, mowing the lawn, cleaning the house.... It's a hard decision to make, my cardiologist said in 2/22 that if I did not take the surgery step, "Your family will miss you." Then it became an easy decision even though I was terrified. Good luck in whatever you decide. You would be in good hands at Mayo. Keep us informed on how it is going.

Yes, my life has improved since my alcohol septal ablation & ICD implant. However, I now have cardiac amyloidosis or stiff heart syndrome.
