GFR dropped 5 more points and I don't know why.

HI all, I got some blood work today and I was actually expecting the results to be good, or at least stable. My GFR dropped from 59 to 54 in 2 months. I have lost 20 pounds and my sugars are within normal limits. So sad.

I have a low GFR (46), without corresponding creatnine numbers and I am not diabetic. The kidney Dr and the PCP have encouraged me to get better about drinking plenty of water--particularly during this really hot summer. Kidney size is ok per ultrasound.

You are only in Stage 3a, as am I. Have been told by visiting health people that I am in a good place.

Me, too! Just trying to push water better than I have until now at 70. It still felt like a huge "whew" when they told me. I had to stop anti-inflammatories, but it wasn't the problem I anticipated with that change. Thanks.

I recently experienced the same situation where my eGFR dropped 4 points in 2 months. I exercise 7 days a week, have a strict diet aligned to my CKD and lost 5 pounds (on purpose). My doc also feels it is a fluctuation that may be related to hydration related to my heart meds used to treat AFib. I drink 1.5 liters of water per day. I am getting retested in a week to see if this a one time event or a trend.

I had completely normal blood word on 11/30/22 except sodium was low and had a UTI with fever. Never had that before and I’m 70. Doctor wanted to recheck my bloodwork last week April 2nd 2023 and shock of shock I had stage 3a kidney disease. Sodium was back to normal but high potassium, GFR 59 and A1C 6.2, had been in the normal range!! Blood pressure 157/105 was normal! How does this happen in 4 months! Have always had normal blood work, blood pressure and A1C!! Something is not right, any ideas? I have a low sodium diet, don’t like salt and eat no sugar or carbs!! Never have sodas or any drinks other then black coffee, water and red wine,

Stage 3A at age 70 is not a death sentence, but a warning sign to be cautious for your remaining years ahead. Unfortunately, in my opinion, blood pressure of 157/105 is not normal. BUT, blood pressure can change throughout the day. When I see my Mayo nephrologist, he/she checks my blood pressure 5 times to get an average. You may wish to check your own blood pressure at a certain time every night to get your trend using your own blood pressure cuff.

Your potassium is high which is another sign of CKD. Be aware of labels with your food intake. You may also wish to see a dietitian who certainly can help.

Lastly, get your blood tested more frequently than 4 months, and watch your eGFR trends. My PCP orders mine every 6 weeks.

Caffeine in coffee is a diuretic for your kidneys. Alcohol is not recommended with CKD I have read.
Also, my personal tells me that overall hydration seems to be the key for me. It does effect the eGFR in my experience. I have recently been diagnosed ( last year) so i have not gotten a handle on keeping the numbers level yet.

Caffeine in coffee is a diuretic for your kidneys. Alcohol is not recommended with CKD I have read.

Thanks for the info! I drink a lot of water. I’m thirsty all the time but then up several ones a night in the bathroom! I would like to have a different lab do my bloodwork it seems they have different parameters.

Thanks! Good to know about blood pressure! I do check mine and is much lower and up until recently it was low at doctors appointments also! I can check it 3 times in a row and get different readings! Are you on meds? Getting married in June, worried about weight gain for my dress… has that been a problem? Thanks

@collegeprof Yes, as you mentioned, and I like to echo, each person has to be their own health advocate, as you know yourself best. Relying on trusted online sites like you mentioned, adding in Cleveland Medical Clinic, WebMD, and rsnhope.org [Renal Support Network].

When you have multiple conditions that coexist and need attention, it's important your entire medical team is onboard with great communication. Sometimes we as a patient need to remind them of that and lead the committee! I changed my nephrologist about a year ago when I felt he was not putting my best care as a priority; my new one, within the same practice, is a great gal who specializes in advanced kidney disease cases! She communicates regularly with my oncologist and vice versa; unfortunately they do not share the same medical records system.

@gingerw can you as the patient request that all your records be shared with both of your physicians (since the medical records systems are different)? You want to make sure they are “on the same page”. After all “you are the customer and the customer is always right”. Besides these are your records and you should be able to share them with whomever you wish.

Actually, I agree with the idea to have a different lab do your bloodwork so you can compare the detailed numbers. Labs do have different parameters.
I only drank one huge cup of coffee per day, but i did find that black tea with cinnamon stick wakes me up and i drink only @ half a cup. I know it has caffeine but I am thinking in much less quantity than how i drink coffee.
I too drink lots of water with fresh lemon in it.
I am going to ask my doctor for testing every 6 weeks as someone here said their doctor did. I have had 4 blood tests over last year and the eGFR is erratic- I am still in state 3a CKD though.
I am 75 and last year was the first time this issue has come up for me, so i do plan to learn as much as I can about my body and hopefully keep CKD in stage 3.
I am not diabetic and only take small amount of BP drugs, but i am in obese category.
We all need to face our health problems and do as much as we can to learn how to keep ourselves healthy as possible.
