Gout meds to lower uric acid and chronic kidney disease

I am Stage 4 CKD and have had a few episodes of gout. Is anyone on this forum who is in late stage taking or has taken meds to help with lowering uric acid? Thank you for any responses, positive and negative.

@kitmus I am Stage 4 CKD. I take allopurinol at night. More important is a diet that eliminates shellfish, pork, processed meats, purines [like mushrooms], and minimal sugars. Balanced fluid intake is important, as is low sodium diet.

@kitmus Drinking enough fluids for you. Some drs now say we should aim for 100 oz of fluids per day. Drink before you feel thirsty. I am known to always be drinking water, even keeping a bottle next to the bed "just in case". Hydration levels are important!

Hello @kitmus. I have CKD stage 3B and gout. Through Mayo telemedicine I was given allopurinol and colchicine. It appears that these are the standard prescriptions. I was also given prednisone for acute attacks. Due to other medical conditions, I had issues with the allopurinol and colchicine. Then I was prescribed Uloric, but still had a reaction. I finally got my Covid vaccinations, so I am returning to Rochester in May for testing, examination, and treatment. I was told that the plan is to reduce uric acid below 6, because that is the level at which the crystals that create the gout are formed. I hope you find this helpful.

Thanks for responding. I am also on blood thinner so have to be careful. It is good to know that docs can subscribe the gout medications to CKD patients. Sorry to hear about your side effects.

I am Stage 3B and was having 3-5 gout episodes a year a few years back, even before I found out about my kidney issues. Allopurinol and colchicine were discussed but I decided to try and avoid them if I could. I did quite a bit of research and put together a protocol for myself that has worked great. My episodes now are about once a year for which I take prednisone immediately and also double my protocol. My episodes have a duration of just a few days and get nowhere near the incapacitation of episodes of years ago where I literally could not walk and used crutches or a scooter. I used to have to take time off work and haven't needed to do that at all. My doctors are very happy with the results. I hope it works for anyone that might try it.

Bilberry Fruit Extract 1000mg 1 x day
Celery Seed Extract 1000mg 1 x day
Bromelain 500mg 1 x day
Ginkgo Biloba 120mg 1 x day

Hello, @gladiator14. I can understand and agree with your reluctance to put more chemicals into your body. Your protocol is intriguing. How did you come up with those specifics?
Based upon anecdotal information that I’ve heard and read, I take Knudson’s Just Tart Cherry juice, frozen cherries which I microwave to thaw, Fage dark cherry yogurt, and cherries when in season. I don’t necessarily take these every day. I drink morning coffee, then water or other non-alcoholic and non-carbonated liquids all day long.
I read that gout may be caused by chronic kidney disease. I have heard differing medical opinions on whether gout can lead to kidney stones. There are also differing opinions on whether vegetable purines, as opposed to meat purines, should be avoided. Additionally, two recent studies question if there is even a connection between uric acid levels and gout.

Sorry for my delay in responding just haven't been keeping up. As far as how I came up with the protocol. I have a Master of Nutrition degree and I spent a lot of time looking at the research. Your taking the Knudson's is fine but I have read studies that the Montmorency type of cherry from Michigan is the most beneficial based on nutrient content. I use Obstbaum Orchards concentrate which is much cheaper and can be bought online. It also has a longer shelf life and I just use it full strength in smoothies or it can be reconstituted back into juice by adding water. I know from my experience when my uric acid levels were 14-16 I had many more gout episodes. Touch wood I am now 6-7 and can't remember my last episode. So I do see a correlation with me as far as uric acid levels. All in all the protocol helped to start but when I found out the CKD and started to limit my proteins, especially from animal sources which I still eat 3-4 times a week, and reduced my sodium things really improved. Also my eGFR and creatinine levels have improved as well.
