MCI with small vessel ischemic disease: Leqembi good option?

My wife is being treated for MCI the last two years. Her MRI shows memory loss with small vessel ischemic disease. She currently is taking 10mg 2x/day of memantine. Her PET Scan in May 2026 showed frequent amyloid neuritic plaques and her Centaloid level was 108.2. She had APOE genotyping bloodwork done in July that showed E3/E4 genotype & positive for APOE E4 which reflects increased risk for late onset Alzheimer's Disease.
Her MoCA (Montreal Cognitive Assessment) score is 19/30 A MOCA which indicates mild cognitive impairment or possible early dementia, suggesting significant deficits in one or more cognitive domains and warranting further comprehensive evaluation and medical assessment. They say the normal threshold is 26 or higher on the Montreal Cognitive Assessment. Her neurologist wants to start her on Leqembi infusion ASAP. Is this a good option for her (us)? Thanks.
