P1NP number on Tymlos

After 3 months on Tymlos, my P1NP bone marker has increased by 50% to 109. I was initially excited, but am wondering if this is excessive. Anyone else had numbers like this?

@willowmena I was 66. When I have my dexa in june I will have been on HRT’s for 1 year so I’m very curious to see my results.

@lurawilson Thank you for your reply. I am asking my gyn about HRT, and my endocrinologist for testosterone cream (previously gave to me but side effects were unpleasant, before I knew I had osteoporosis). I will try it again if I can take the 2 together, along with my Tymlos and vitamins. Can no longer do weight resistance exercise due to my various back/neck injuries and too much pain. I am at the point of investigating all viable alternatives!

@willowmena I hope you can start HRT and respond well to the Tymlos. I will report back in June with my DEXA and bone markers results 🤞

@willowmena I was 66. When I have my dexa in june I will have been on HRT’s for 1 year so I’m very curious to see my results.

@lurawilson Did you get your June DEXA? Any change on HRT? Were you on any osteoporosis drugs?

I just saw this old post. My P1NP went from 74 to 147 in one month, but 119 at month three. I was doing weight training, for month one and two. I stopped exercising for month three, due to an injury. I am training again, and hop it goes back up for month four. If you continued to track your P1NP over the two Year on Tymlos, could you share that info? Also, how did your DEXA improve over the two years on Tymlos? Thanks!!!

@summera1 yes I got good news for my lumbar spine and thanks for reminding me to post an update. My lumbar spine went down to -2.2 which is even lower than what it was in 2022. However they did see a compression fracture at T8. Here are my scores
2022
Lumbar spine -2.8
R hip -2.5
L hip -2.9
2024
Lumbar spine -3.3
R hip -2.6
L hip -2.6

Lumbar spine -2.2
R femoral neck hip -2.5
L femoral neck hip -2.7
Forearm -1.8
Total R hip -2.1
Total L hip -1.8
This is the first time I got total hip readings and femoral neck. My bone markers also improve. I have been on Tymlos and HRT for 1 year . I plan to stay on the Tymlos for another 6-12 months depending on my bone markers and then I will get a reclast infusion and take a drug holiday but stay on HRT indefinitely. Hope that helps!

@lurawilson Thank you for your kind reply:) My p1NP went from 74 to 119 and CTX from 946 to 665 for three months on Tymlos. I get a DEXA in two months. I would like HRT if it helps bone, but trying to see if hyperthyroidism or high LDL levels would be affected. I was on bioidentical topical (thigh) cream with estrogen/progesterone combo and felt wonderful. They made me stop after ten years, but now they seem to be allowing it again. However, I stopped taking them about ten years ago and now they saw I can't start again because it has been too long. My doctors don't seem to want to give me HRT but I am still researching. What HRT form do you use? Pros/cons? Any advice on convincing my doctors?

@lurawilson, your results are wonderful, especially thr great improvements in your hips. Congratulations!

I read your previous posts - are you still on the same HRT dosing with no change? If you are comfortable sharing, what are your latest BTM readings? I'm very curious about estrogen's effect on the markers when used alongside an anabolic like Tymlos or Forteo. Thank you!

@summera1, you wrote "CTX from 046 to 665 for three months on Tymlos": was 046 a typo? If not, were you on a bone drug at that time?

@mayblin Thank you, I am really thrilled with the improvements in my hips as well! To answer your questions, yes, I am still on the HRT regimen: 100 mg progesterone daily and the 0.0375 mg estradiol patch changed twice a week. My most recent bone turnover markers from June were:P1NP: 89 CTX: 493
