RA, methotrexate and Anemia recently diagnosed

Posted by juj @juj, Feb 25, 2023

I have rheumatoid arthritis and have been taking methotrexate since July 2022 with 2 infusions of Rituximab in January. I have blood drawn every two weeks with my baseline established June 2022. My RBC, hemoglobin and hematocrit have been declining with each blood test and all three results are out of range. I recently had bloodwork for Iron TIBC, folate, serum, vitamin B12 and ferritin and all results are in range. Does anyone have experience with this or something similar?

This is exactly what’s happening to me. My PCP is saying anemia of chronic disease but I’ve only been diagnosed for & had this going on for less than a year?!? I’m tempted to treat myself with some OTC iron. My rheumatologist has not commented yet.

@kelsa
May I suggest asking your rheumatologist, or PCP, for a "Full Blood Count (FBC/CBC)" to check hemoglobin, Serum Ferritin to measure stored iron, and Serum Iron with Total Iron Binding Capacity (TIBC) to measure circulating iron.

Serum Ferritin: This is considered the most important marker. It shows how much iron is stored in the body. A low ferritin level confirms iron deficiency anemia.

No, but I was on methotextrate for 20 years with no problems didn't even know I had RA. No symptoms but I had to change in the last month to Rinvoq because methotextrate gave me too many flares this last year and a half. Good luck
