HRT Safety

I wanted to share this Medscape article I received today about new study on HRT. I am considering HRT for my osteoporosis at age 67 and am so encouraged by this article: https://www.medscape.com/s/viewarticle/hormone-therapy-after-65-good-option-most-women-2024a10007b2

@dvargo Can I ask what type of HRT you are on? Do you use topical gel, progesterone pills, testosterone? I'm learning about this now at 63. Thank you!

@bano Topical estradiol and testosterone. Progesterone is a pill. I started at 64, now 67. My bone density increase 5% in one year.

@dvargo Wow that's impressive about your bone density increasing! Can I ask how much estradiol, progesterone and testosterone you take? I realize we are all different and I'm trying to find the right balance which I don't think I have yet.

@dvargo Thanks - very interesting about your testosterone and progress. I had some self-pay tests done recently and I did not check my testosterone because I did not yet understand the importance of it. Video linked below is influencing me which has a key message is that one needs to measure testosterone to see whether you need it.

I am 6 months on the same HRT as @kshea2 above: estradiol 2x weekly 0.05 patch and 100mg micronized Progesterone, prescribed by my great PCP.

Hi All - I haven't posted in a while, so wanted to share my progress after 1 year on HRT. Some background; I just turned 63 and had a T12 Spine fracture in December, 2024. I was lucky to find a PA at my clinic who was open to prescribing HRT. I'm sharing a lot of data below and hope this helpful to others. I'm am VERY encouraged by the increase in BMD per the REMS scan and really hope this is accurate. I'll have my next DEXA this fall on the same machine and add the TBS score.
One interesting point is the difference in my last DEXA (osteoporosis in Spine/Osteopenia HIP) and REMS showed the opposite. Also, my first CTX/P1NP might have been falsely high due to 5 months post fracture. My 2nd P1NP is lower than I had hoped but my research using AI indicates the drop is normal on HRT.

I wasn't able to paste the excel data here and keep the formatting, so attached a pdf copy. Hope it works!

@kshea2, thank you very much for sharing your experience. I am very sorry to hear about your fracture, and hope that you’ve recovered well.

The preliminary positive REMS results are very encouraging! Like yours, my bone markers (both CTX and P1NP) have decreased, though perhaps more in magnitude (i'm finishing up my second year of HRT currently). This is exactly what is expected for estrogen, as it acts mainly as an antiresorptive.

Wishing your upcoming DXA a similarly positive outcome later this year and continued steady progress.

I am 54 and have been on HRT for over a decade. My dexa scores have drastically declined. I am now a -4.4 in my spine and -3.1 in my hip. I have taken other osteoporosis meds in the past and have just had my first dose of evenity. My doctor said that because I went into early menopause at 38 and because my mom has osteoporosis, is why I have it. I tried to go off osteo meds for a while and just do HRT, strength training, supplements and diet. It did not work and now I am paying the price.

@kshea2 These numbers are very encouraging and validating of my own experience on BHRT started at age 67. I also gained close to 5% after one year. I am due now for another REMS scan so will see if I've had any further gains after two years on BHRT. Thanks so much for sharing your numbers.

@dvargo , Would you mind sharing what type of machine your bmd was measured on? Was it a DEXA or REMS? I had a similar gain on BHRT after one year measured on a REMS, so am curious to know if a DEXA would show similar results.
