Tymlos side effects

Posted by loh @loh, Jan 29 8:25am

Has anyone else had leg/back aches with Tymlos? I've been on it since October and lately I've had terrible aches in my legs and lower back, especially at night. Is that one of the known side effects? Thanks!

After 3 months I still get a headache after my injection. Sometimes really miserable and sometimes it is minimal discomfort. For me this whole treatment for osteoporosis is really difficult on our health.

@robe1018,
hi. the endocrinologist I see thinks that the whole headache syndrome with Tymlos is because of vasodilation of the blood vessels. The medication works because parathyroid hormone we inject tells your body that you are low calcium. It sets off reactions that pull calcium back from your kidneys, draws it from your digestive tract and from your bones and into your blood. Drawing calcium from intracellular spaces relaxes blood vessels. Gradually over the serum calcium returns to normal, but meanwhile,
widening blood vessels in the head stretches pain receptors there and can stimulate the trigeminal nerve to produce chemicals that cause migraine-like throbbing.
All the medications we take (for everything) set off chemical chain reactions unrelated to the illness and often unknown even by our professionals. Our bodies make the discovery sometimes unfortunately for our health.
A single study determined dosage. https://link.springer.com/article/10.1007/s00774-023-01455-6 which would tell us how many were enrolled. In the study side effects were said to be not dose dependent. I don't have access to the study itself.
The Tymlos pens are of a fortunate constuction--maybe intentional.

I have been on Tymlos for one year now. Bone density shows an improvement in bone strength in both hip and lumbar back. I, too, have had varied side effects namely the tachycardia. I even went to the er one night due to increased heart rate. I have experienced this now 3 or 4 time. Leg pain, headache, etc. are all symptoms are all ones I have experienced along with some bloating in my stomach at times. All of these symptoms subside over time and for me, I prioritized gaining strength in my bones over suffering the symptoms. This is an individual decisions for each of you of course.

I don’t know if this will help you or not but if you are not already…
1) Be sure to hydrate throughout the day with fluids (not water it flushes out electrolytes from your system) Try a half cup of prune juice, standard green Gatorade 50/50 water not too sweet.
2) If you drink coffee or something you can add a little to and you don’t have a sodium problem add a 1/8th teaspoon (very small amount of table salt) to it.
3) Take a couple ibuprofen with food like an English muffin with milk, non fat if you can ( calcium) if your doctor says it’s okay…it helps constrict (vasoconstrict a little)
4) Make sure you are getting recommended amount of calcium and d3 you need. Everyone is slightly different. But some say don’t load up on supplements just before bed …take through the day spread around.
5) have your blood checks for markers that can cause headaches…talk to your doctor…
I tried this and it worked for me…

It’s possible…doing a few things worked for me…no headache this morning…upon waking.

@mamabear77 I had no idea that was even a side effect (doc never mentioned it) until I saw others here talk about it. The first side effect I noticed was severe head aches, but that went away after 2 or 3 weeks. The leg pain I endured for about a month I think. I couldn't get comfortable either, I was basically writhing in bed because my legs hurt so much. I felt so much better when I stopped, no more leg aches and I realized how tired and overall bleh I had been feeling for three months. This medicine has to be pretty serious to cause so many serious side effects. I am probably being foolish going off all medication, but I hate the idea of going back on.

I tried Tymlos (starting in July 2025) but consistently had headaches and my heart rate went up to 90-over 100 BPM for at least 1-1/2 hours after taking the medicine. I’d still wake up with the headaches as well. I went down to 2 clicks to see if the side effects would subside but they didn’t.
I ended up stopping Tymlos after 5 weeks of the consistent headaches and elevated heart rate. I am 66 years old and have severe osteoporosis in my spine but the side effects were too severe.
I changed over to Evenity and have had 2 months of shots already with no side effects. So far I am much happier with Evenity.

I got injected again 7:30 pm Friday night 11-14
Had the same post heart rate increase for about an hour then it started to get lower by 9pm (1 1/2 hrs)
Awoke again with headache about 4 am accompanied by some neck pain this time. A little worse this morning…but feeling a little better now after moving around…it’s 6 am

@osteopatient2026 5% of patients get palpitations. My Dr took me off. But I resuming it this week.
She suggested getting a device that does an EKG or Fitbit watch ( the EKG device is only $80) To monitor heart to see if it’s AFIB or just beating hard. I’ll be looking for a device. Or if you have a cardiologist they can put leads on your chest and monitor your heart for a week. If this gets too bothersome for me, I might change to Evenity. Warm wishes, E

Thank you so much for responding @windyshores. I’m actually feeling less fatigued and feel less of the increased heart rate as I’ve ramped up to six clicks. I had a normal nuclear stress test which was reassuring. I will try taking Tymlos in the morning to see if it’s better.

Buuuuuuttt — now my right knee has been killing me for a week. This is after about 20 days of Tymlos. There was no event to cause this. It’s confusing because 1) I’m overlapping with last three months post Reclast and 2) I do get various joint issues. But my knees haven’t been a problem for years! My hands ache but I can live with that. It’s hard to put up with pain that limitS movement — now I’ll be getting a MRI ETC. I’m going to put up another question about this to see if it’s happened to others here. Thank you again!

@jessbricht
I found I had side effects/pain in body parts that were previously compromised from injuries. I was a marathon runner, ice skater and rode horses (I was thrown more than once from horseback, took some major falls on the ice, and fell several times running, injuring my knee). All of the body parts that were previously injured years ago woke up and complained after I started Tymlos. The headaches were short lived, but the aching legs and hands have been ongoing. My damaged knee throbbed during the night. Recently the stomach upset began. Of course, that's because I have chronic pancreatitis. I just stopped the meds for several days (was on it since Sept. 27) and the symptoms abated. But with osteoporosis, meds are needed, despite my years of athletic undertaking -- now no longer in the last 10 years. I am 76 years of age. I think the side effects come and go.

Thank you posting that. What are your T Scores? I’m sort of in the same situation. I had one morning with no headache but after 4 days on Tymlos it’s back this morning. I know it can be a side effect up to several weeks but for most “I’ve read” the side effects should subside.
I went on Tymlos because I might need a tooth extracted and implant.
Prolia was the other drug recommended but it doesn’t build bone as fast but some studies show it continues building bone over time (years)

@osteopatient2026 Prolia does not build bone, it only prevents further bone loss. I would avoid it. It can also make you lose bone faster when you stop it.
