Castration Resistance

For all of us that are having success with ADT, I’m sure there is the dread of hearing the words that our prostate cancer has become castration resistant. I am on month three of Orgovyx and Nubeqa. I asked my oncologist if he could give me some kind of estimate as to when hormone sensitive PCa becomes castration resistant and his reply was that he couldn’t. It’s different with everyone, he said. That being said I’d love to hear stories of your journey from hormone sensitive to castration resistant. Things like timelines, treatments, etc…anything that you might find helpful to share. Thank you all in advance. This forum has been a wealth of fantastic info for me!

Sounds effective. What ADT, ARPI medicine or medicines are you on?

@jeffmarc Hi Jeff, I'm assuming you're not metastatic since you were allowed to get on Nubeqa while being castrate resistant? Thanks!

I became metastatic before I was put on Nubeqa. My oncologist told me that since we’re in an HMO, she is not limited to FDA approved treatments. At my request, she put me on Nubeqa, I didn’t realize at the time that FDA had not approved it for exactly my situation.

I know a lot of other people that have been able to get it without a problem, even though they were mCRPC.

@abcsoup My husband has had Stage 4 metastatic prostate cancer for many years and is castrate resistant, and his medical oncologist at an academic medical center recently recommended that he begin Nubeqa. The doctor was able to get prior authorization for my husband to take Nubeqa without any trouble in spite of his disease status.

My father started Nubeqa with Eligard in November of 2024 for omHSPC and recently began a treatment holiday due to his PSA being below 0.1 for the last 16 months. My only concern lately has been the matching-adjusted indirect comparisons between Xtandi and Nubeqa showing that Xtandi increased time to progression and castrate resistance significantly. If and likely when he's due to resume treatment, I will be asking about whether to stay on Nubeqa or perhaps switching to Xtandi.
