Unidentified abdominal pain

I wasnt sure where to ask this, but I've had severe abdominal pain for 16 years. Pain radiating from my back all the way around and sharp upper right abdomen pain. Lasted a few hours-a day for 16 years a couple times a year. Then recently i had an episode that went off and on for two weeks straight. Now when it happens theres pressure all around my abdomen and back, and sharp pains randomly around my torso, no sharp pain felt when they push on my abdomen, all blood and urine tests good, xray and ultrasound show nothing. Doesnt seem to be linked to eating seems to happen at random. Anyone experienced anything like this?

My test results just came in for the MRCP and HIDA and are negative to remove my gallbladder. Eight months ago they said there was sludge and an enlarged gallbladder, that seems to have gone away? I have significant daily abdominal pain, can’t eat without pain and gas. I don’t know where to turn for answers and feel this is not sustainable. Any thoughts about other diagnoses? My doctor doesn’t think it’s MALS

I am lucky to have a GI Doc that is knowledgeable about available tests and supports using the dietitians in his office. He does understand how much of a role food intolerances and enzymes play in gut health. Some docs just run tests to rule out the big stuff like pancreatic cancer, etc and stop at that.
I have an RX for Levsin when my gut gets angry. But it has taken almost 30 years of having the broad diagnosis of IBS to get it figured out. I tend to do a lot of research myself and go to the doc with a list of questions. Hope they figure yours out. I’d definitely start with a food journal and eliminate some of the main irritants like caffeine, alcohol, dairy, sugar, gluten, etc. and see if that makes a difference in how you feel.

@drlori76 It is frustrating when modern imaging technologies fail to detect what the problem is. What were the findings on the scans? "Negative to remove my gallbladder" doesn't necessarily mean "normal".

My daughter and I had HIDA scans that showed "sluggish" gallbladders, but NO stones or sludge, thus questionable for removal. We both had surgery due to the symptoms, and the pathology findings were chronic severe inflammation. I was fortunate that my primary & the surgeon practice in the same clinic, and she told him to "trust Sue's instincts" so he did the surgery. I no longer have all the symptoms & pain.

What does the doctor suggest as the next step?

@sueinmn I’m so glad that you and your daughter got the care needed. ❤️🙏🏻 I have a sludge as well and lot’s of symptoms I’m fact I’m severely ill now. My CA 19.9 is increasingly high and I have NET rectum history. I’m waiting on HIDA, barium swallow and SIBI on other center. I had been asking for further investigation for the longest… it has been very difficult to advocate for myself lately specially with all my symptoms (on my other discussions).

What should I’ll be looking forward to in this process? I have all the evidence but it’s really hard me to get a comprehensive plan of care.

Any feedback would be greatly appreciated 🙏🏻🌼

Sorry you going through all that
The HIDA is a long procedure but not bad. They inject a dye in your veins. You wait about 45 minutes then do imaging for an hour.

My test came in low for remove my gallbladder , because of the pain I demanded to remove it. They said the problem could be that I have high iron in my liver. I had it remove red 5 days ago . My problem is a bad pain in my back

May I ask, did you find a diagnosis for this pain? My wife and I are seeking advice/support/diagnosis on her undiagnosed abdominal pain. Her words are pasted below:

'I am suffering from severe abdominal issues. I have full and constant pain that flares up in my lower abdomen. The pain is severe and makes me bed-bound for days. The triggers are caffeine, intercourse and orgasms. I have been suffering for several years now but nothing has ever been identified by my Doctors and it is becoming more frequent and more intense. My life and therefore the lives of my family are being severely affected.

Endometriosis and adenomyosis have been ruled out.

I have had a laparoscopy, MRI, CT, cystioscopy and ultrasound have all come back clear.

PLEASE can anyone help? I am so desperate. Thank you.'

You might also have them check for C-Diff.
My abdominal pain eventually turned into C-Diff, which I
discovered was actually life threatening.
A physician specializing in rare diseases finally helped.
I still wake up with some abdominal pain & just take a tramadol each
morning. I also have had bladder infections that appear as
abdominal pain.

Since 2021 I have had severe abdominal pain, can’t eat or drink water for having severe pain around my belly button, actually if take a vitamin it hurts. I have many Ct scans, X-rays, 3 colonoscopies, 3 EGD’s, Colonoscopies showed I had polyps 1 in the transverse and 1 hepatic flexure. They were tublar adenoma. The EGD that I just had a few weeks ago reveal several polyps in my stomach and inactive gastritis and inflammation. But I have very few bowel movements and I have had blood! Colonoscopies always show unremarkable, Some thing is being missed I have been living with 3 1/2 years, it just seem like doctor don’t care figure out what is wrong. Please any advice I appreciate.

Sounds like you've also had an upper endoscopy. Is that when the stomach polyps were discovered? John's Hopkins has a fantastic gastroenterology department. Whe a large polyp was discovered in my duodenum I was very lucky to get an appointment quickly for removal. I was even luckier to get Dr. Mouen Khashab as my doctor. I wonder if they might help. Are you on original medicare with a supplemental?

Actually, I am not old enough yet to have Medicare, I have a few more years. I am so tired of abdominal pain! Seem to now hurt all the time! Thanks for your reply!

Have you been breath tested for SIBO? Those are the symptoms I had. I have had to go on a gluten free, low FODMAP and limited dairy diet. It works well.
