Reclast Infusions: Side-effects & Recovery time

Posted by cindydee @cindydee, Mar 20, 2018

I just had a reclast infusion last week and have had serious side effects. I had the worst flue like aching for 5 days then my lefty arm became full of inflammation in the wrist, elbow and shoulder which caused extreme pain and I lost the ability to straighten my elbow. Ultrasound showed huge amounts of fluid throughout the arm. The right arm is now starting to have the same symptoms. The pain is excruciating. Has anyone else experienced anything like this? Neither the ER doctor nor the Dr. who prescribed the procedure knew what to do to ease the symptoms. Both arms from fingertips to shoulder are swollen and neither elbow will straighten. Anyone else have adverse reactions to the reclast infusion? If yes, how long did it last?

Hi. I had second reclast infusion 3/17. I have had prior acid reflux issues and am 74. The day after infusion, I threw up breakfast and experienced stomach pain. This has gone on, off and on, for 12 days. Dr. gave me a anti nausea prescription which kept me from throwing up, but did nothing for stomach pain. I've lost 10 lbs., gone to the ER once, talked to the health care nurse twice and my doctor once. I'm trying to eat one ounce of food every hour or so, but still have stomach pain. I'm sleeping a lot and and hoping symptoms don't last much longer.

I just saw your post from Sept, 2022.

How are you doing? Has the pain level reduced?

Hope you are feeling better!!!

Yes, me, too, I just saw that post and hope she is feeling better. You just never know sometimes especially if it's not a doctor. That's why I'm here trying to research about green lipped mussel oil wondering if it really helps. Don't want to waste my money.

I feel for you with your symptoms! I have awful shoulder pain, foot pain and periods of upset stomach. You said you seem to sleep a lot! I’m not a doctor, I too often feel exhausted. I make myself go for a walk with my two fur babies because I know very well that Stress can be exhausting and take over my life; it has and I will not let it happen again! I do hope you start feeling better and not so exhausted

I had my first Reclast infusion a year ago with no side effects. However, after another bone scan it shows I actually got worse. My doctor still wants me to proceed with another infusion. Has this happened with anyone else and is it worth it to continue.

Hello, all, I had a Reclast infusion on Dec 18, 2024. I drank a lot of water before and after and took Tylenol and had no real known side effects. However, I quickly put on 10 pounds through the holidays. I have not been able to get this off. Would like to know who else this has happened to. I am only 5 feet tall so every 5 pounds makes a difference and 10 means there are clothes I can’t wear now. I’m so flabby now and the regular exercise I did before doesn’t seem to help. I have a specific diet I follow. Have kidney stones too so there are some foods I can no longer eat. Just wondering if others have had this issue on Reclast and if it ever resolved? Saw my oncologist yesterday and she wasn’t aware of weight gain being an issue. Thank you in advance.

I had my first Reclast infusion a month ago. First day was fine, but the following night I had severe foot and calf cramps for about four hours while trying to fall asleep.
Five days later, my wrists swelled up to the point I had limited use of my hands and was in a lot of pain. After three days of ice/Advil, I went to Urgent Care and the PA prescribed prednisone. Not exactly what you want to take when trying to improve bone density, but it brought the swelling down as well as the pain.
Now my only issue is really noisy joints. Both wrists, my left shoulder, and sometimes my knees crack when i move them. It isn't painful, but it's a bit annoying. I actually had the same issue with noisy joints when I was a child, so I'm not terribly worried about this...just annoyed by the noise when I make certain movements.
The PA recommended discussing with my doctor about the possibility of taking colchicine prior to the next annual infusion, suggesting that the issue could be Reclast-associated pseudogout.

I had a Reclast infusion in Oct 2024. I had no immediate reactions. However, in about a week after the infusion I had the acute onset of my right knee swelling which was quite painful. After some pushing of the system I had an MRI of my knee and 60 ccs of fluid removed from my knee joint. The fluid results show calcium pyrophosphate crystals as seen in pseudogout. On the MRI I had a large joint effusion with fluid tracking into my calf leaking from the joint space above. Removing the joint fluid helped tremendously with the pain in the knee and calf. After the diagnosis was made I had a steroid injection into the knee joint which helped some for 1-2 months. Then I had further progression. Another 55 cc's of fluid were removed. Still pseudogout. Later I saw a rheumatologist who put me on Colchicine 0.6 mg a day and gave me another steroid injection into my knee joint. At that time I had right and calf ankle edema from the joint effusion compressing what is called the popliteal vien, which runs behind the knee. The compression of the vein decreases the blood flow from the lower leg to the upper leg, sort of like a dam. The steroid seemed to help with the pain for about a month. The ankle edema resolved. I do not think the Colchicine has helped. After about another two months my knee joint blew up again. I had 80ccs of fluid removed. Again there were calcium pyrophosphate crystals. I am quite miserable. It is now 7 months since the Reclast infusion (actually the generic version). I am frightened that this may not resolve. It has been a significant disability for me. I have had one continuous episode with little resolution, not episodic events with complete resolution between bouts. The pseudogout seems to be causing a lot of damage to my knee. Pseudogout is not a commonly reported side effect of Reclast (or generic version). I am wondering if it is indeed more common than is reported because the diagnosis is brushed aside as unlikely.
I am posting this so that the previous writer, analchemist, knows that Reclast associated pseudogout does exist and is not a good complication. I am also interested if anyone else has experienced these symptoms.

I’m so sorry- I had a kinda similar experience with Reclast that thankfully resolved.
About 3.5 years ago, I fractured 2 vertebrae (1 mild, 1 moderate) skiing, I took Evenity injections, then a Reclast infusion to hold the gains.
I had the usual fever and aches ( and also got covid from the clinic I think), about 4-5 days later my right knee swelled up suddenly!
I could feel lots of fluid around it, was very painful where I couldn’t put weight on it. My endocrinologist said just ice it (RICE) and let her know if it doesn’t resolve. It did after about a week and hasn’t been an issue since.
I generally am back to normal but feel more joint pain I think.
Its been 2.5 years since and no more drugs.
I just got a dexa-scan so curious how I am doing. I am 66 now.
I have been going to Osteostrong the past year. The only other medication left is Tymlos…

Thank you for your reply. I am glad your right knee swelling resolved.
I cannot find anyone who seems to be able to help me. I will keep trying though.

I think there are multiple causes of arthritis (joint swelling) after reclast and a swollen knee may be due to CPPD (pseudogout) with a risk of recurrences or may be an inflammatory arthritis without the calcium crystals which is less apt to recur over time. @cccs you may have had the latter.
