HoLep vs TURP

My husband is scheduled for prostate surgery early Nov. The doctor has agreed to do his cystoscopy under full anesthesia right before surgery… this is because the first time my husband had a cystoscopy he was fully awake, and it was very painful for him. However, the doctor stated that he would have to decide at that time whether he would perform the TURP or HoLep. Can someone advise the differences between the two and benefits/cons also between the two? What determines which procedure the doctor will go with?

Given the choice go for the holep. It's considered a one and done procedure.
TURP tends to leave tissue which can regrow. Thus, a second procedure down the line possible..
Your prostate is like your nose it continuously is growing.

I would get a second opinion. I had a cystoscopy in June and there was no pain. Need to find a reason your husband had so much pain. Why is the doctor doing another cystoscopy? Turp is old, HoLEP is new and can be used with any size prostate. I would look for a doctor who is willing to answer your questions so you don't need to come here. LOL

The best thing for both you and your husband to do is research both options and read as many reviews as possible on HoLep and TURP to decide the better option.

This will also help you both formulate appropriate questions to ask your physician and depending on his bedside manners and responses to your concerns you guys can make a better informed decision to decide if you need to switch doctors.

Asking questions in this space as you have done here, is also a good option. People are open and bring forth good insight.

If you and your urologist agree on what procedure will be done based on what they find from the cysto, this may be a solution. You certainly want to know you will get the appropriate procedure for your condition. I would also add green light laser to your options (aka PVP - photoselective vaporization). I have had PVP and HOLEP. If I had to do it over again I would have gone straight to HOLEP. Each procedure has its pros and cons and applicability based on your prostate condition and other personal health factors.

I also had the Green LIght Laser and then HOLEP.......The laser lasted about 4 weeks, then back to blockage....the HOLEP went smoothly, and would recommend this procedure.......

There tends to be a lot of recommendations for HoLep because it's new and effective. However, the best choice depends upon what is found with the cystoscopy, and the person's unique specific situation. For example, if the cystoscopy shows only a median lobe blockage, then a limited TURP procedure to shave the median lobe might be the best option. This particular procedure may not have some of the same downsides as a full TURP procedure, such as retrograde ejaculation. It is also a bit less invasive than HoLep, which takes much more prostate tissue. For patients that also have prostate cancer on active surveillance, preserving the extra tissue may also reduce complications later on once the prostate cancer is treated, if necessary. I would talk with the Doctor about what they expect to find, and what specifically would cause them to go with TURP vs HoLep.

Has anyone had a catheter in for year, and had the HoLEP procedure . What cause your urine retention? I'm looking for answers for my dad who 79 yo. He having this done on the 24th of October? Any pre suggestions for him of what to come after surgery? Thank you sp

Catheter should not be an issue. The holep procedure will open him up just like he had a catheter in place,

I was on a catheter for four months and just had a HOlep. Everything working just fine.
