Radio frequency ablation in liver Mets

Posted by anindita @anindita, May 1 2:11am

Hello Fam..!!
Does anyone have idea about Radio frequency ablation of liver lesion in mets? How effective is that? What is the procedure? Any drawbacks?
My Dad is Ca pancreas patient. He had 12 rounds of folfirinox before surgery and 2 rounds of folfirinox post Whipple's. Whipple's was done in Aug 31,2023. Now liver mets after 7 months of Whipple's. Now he has undergone 3 rounds of gem/abraxane. Now RFA is scheduled after 1 week. Any advice?

I had Y90 to liver mets which is the insertion of radioactive beads directly to the tumor area.
Is this the planned procedure?
In my case most of the tumor bed became necrotic and allowed me to move on to pancreas surgery.

The recovery was very easy. A few days to get over sedation. Home same day.

@gamaryanne If you dont mind me asking, where did you have your Y90 done?

Thank you for the info.
What benefit did you get from this procedure? Did you continue taking chemo after that?

I have a question.
If u had liver nets, that means you were stage 4. And u were still allowed surgery. Is that because no arteries were involved? For me, Chemo and high dose radiation has stopped/shrunk the primary pancreas tumor and no new growth has been seen in several months. I have one very small met on my liver. I am stage 4 but with artery involvement so unfortunately no surgery for me. I will have Y 90 this month because the liver tumor is near my stomach and it sounds like no one wants it to possibly spreading that direction. My hope is this will allow me a break from chemo. Is that realistic? Has anyone had long term success with this procedure?

No I haven’t had but there is a clinical trial I know of on west coast (UCLA) claudin 18.3 protein you must test positive for by tumor tissue testing. I have tested positive for this protein and when my GAC chemo wears off (I’ve been on it since Jan 2024 and cancer antigen is now normal even though a have a liver lesion (metastasis) under 1.5 cm, I will enter that clinical trial which has had good results so far for gastric cancer. Results for current trial for pancreatic cancer haven’t been published yet. I have nodules in my abdominal peritoneum which are stable but dr was unable to get a good biopsy material if it, but they assume it’s cancer since that is a common place it goes. My oncologist in Irvine says pancreatic and gastric and gallbladder cancer have a lot of common traits. Chemo is not the regimen for the clinical trial; it’s an antibody pill. Dr Zev Wainberg up at UCLA can provide you more info about the clinical trial. Best wishes for you.

Hii. My father also had an artery involved during the initial diagnosis. But it had not spread anywhere else. Still most of the doctors had already said no for surgery. But then we met one of the best surgical oncologist in India who specialises in Whipple surgeries. He advised us for 12 rounds of chemo. The tumor responded a bit after 8 rounds of chemo and so we continued for rest 4 rounds after that. After completing all the chemo cycles he went for surgery. The doctor said it was a very difficult case but it did go well.
Usually they say if surgery goes well the remission period is long. But it was our bad luck the disease came back within 7 months of surgery and this time in liver. This time doctor gave us the option of RFA and chemo as treatment.
Rest is upto Almighty!!

Thank you, Anindita! I hope everything goes well with your dad. Did the cancer in liver respond to the GAC?
