Why are urologists dismissive about Decipher?

I have talked to four urologists. All four told me that the Decipher score does not change their assessment. On the other hand, the oncologist and the three radiologists I talked to all stressed it. Why is it perceived so differently?

That’s the nature of machine learning algorithms. They are black boxes. Sometimes they layer ‘reasoning’ over the results but that’s really not an accurate assessment of what actually goes on behind the scenes.

If you have a Gleason seven Or eight And they do or don’t find other things in your biopsy then getting a decipher score can give you a good idea about what’s going to happen with your prostate cancer future. Whether or not a reoccurrence is probable to happen.

If you have a lot of aggressive things in your biopsy it may not make sense to do, since it’s probably going to come back anyway.

If you are a Gleason 9 or 10 then a reoccurrence is probably going to happen, so the decipher score isn’t really as relevant.

In your case, it made a lot of sense to do decipher. You did have a good percentage of cores With 4+3. I suspect they found other things in your biopsy in order to get up to .96

Were any of these things found in the biopsy like intraductal, cribriform, Seminal vesicle invasion, EPE or ECE. (Extraprostatic extensions extra capsular extensions)

Those could mean that your cancer was more aggressive and could explain the .96 with a 4+3.

@service1010 I cannot comment. I didn’t have one since with a 4+4, it would not affect my treatment decision. And I’m sorry, I therefore never learned much about it. Silly, really with a career in AI.

@jeffmarc Thanks for your comment and question. First, they found nothing else on biopsy, except two cores 3+3.
Father died of PCa 50 years ago, older brother RP 10 years ago, younger brother RT last year.
No cribform reported, no seminal vesicle invasion. No PIN. They say 'confined' but PMSA PRT in 3 weeks.
AS for 13 years
Just so odd....
