Undiagnosed for nearly 4 years: nausea and upper abdominal pain, weak

Hi there,
I was a fit and healthy 19 year old, when I was struck with immense nausea and upper abdominal pain, bloating and extreme fatigue to the point where I'm bed bound, and weak legs, which I can only describe as feeling as though they've been filled with concrete.

As the years have progressed, I now have heat intolerance, lesions on my scalp, my heart rate goes up over 40 beats per minute when I go from seated to standing, and yellow bruises that seem to appear on my legs for no apparent reason, and I often wake in the night with a knawing pain in my stomach like I've never eaten before.

I have seen a gastroenterologists who have ruled out crohns disease, IBS, gastroparesis, inflammation in the gut and food intolerances and have discharged me, saying there's nothing more they can do for me.

I'm at my wit's end, and have spent almost four years bed bound. No anti nausea medication helps at all, and the only medication I'm on is sleeping tablets to sleep through the pain at night, although it still wakes me during the night.
I have attached pics of the scalp lesions.
If anyone has any clue as to what this could be I would really appreciate some help as I'll be turning 23 this year, fell ill when I was 19 and just want some kind of quality of life back. Thank you so much, in advance!

@dillyp24 Thanks for asking! Just the same, except all doctors have discharged me. I was diagnosed with POTS but they say they won't be the reason my stomach symptoms are so debilitating. Had a pilcam a few years ago which found intestinal lymphangectasia and pre pyloric erosion, and really thought it might give some clue as to what's going on but they said it was a normal finding. Still searching for answers!

The circular rash on the scalp should be considered. Lyme disease typically presents as a bulls-eye but presents in other ways too. Lyme has many possible expressions, including Gi symptoms. The various symptoms reported; night hunger, heart rate variation, heat intolerance, weak legs, etc., which have no one apparent cause may suggest a systemic issue like lyme. Get tested; rule out lyme, sibo, mold and parasites, etc. Bring results to an ND/ DC with an emphasis in these areas.

I was diagnosed recently with Fatty Liver Disease (non alcoholic) but I have been to the hospital before back when I first started experiencing symptoms, and the doctors there told me I had inflammation throughout my stomach and other parts of my abdomen if I remember correctly. The rest of my organs seem to be fine and I do not have issues with my gallbladder. I also had my appendix removed when I was a baby. The nausea never really leads to vomiting but sometimes it can, and the pain is dull I would say. Please help me out!! The pain is a lot to handle and I’ve been missing out on school and other activities due to my pain and nausea. I just want answers or ideas of what it could be to bring it up with my doctor(s), thank you for your time!!

@soupful It sounds awful going through this for two years, I’m assuming you had tests done to look into the area where you’re feeling the dull pain and nausea? Have you had your stomach looked into, I don’t think it’s an ulcer because the pain isn’t dull, does the pain go into your back, also have you had an endoscopy done? I know that I’ve had discomfort in the same area along with nausea but it didn’t last that long so I feel that you need more tests done to find out what’s causing this problem. I wish I could give you more information but it could be a number of reasons you’re feeling this way, perhaps keep a record of what you’re eating and see if the discomfort increases with different foods, I hope that you find the reason for this problem, best wishes and keep in touch when you get more information.

Thanks for the reply. I have had my thyroid function tested a few times over the past few years. I've had a test for diabetes, Addison's disease, gut inflammation, calprotectin, kidney function, h pylori. I've also had a head CT scan, mrcp and gastroparesis breath test, and a pilcam which showed healing ulcers and lymphangiectasia, though my consultant assured me that this is a common finding, and won't be causing my symptoms as it's just a single one.
I also had a heart rate variability test which showed I have very little parasympathetic activity.

Have you seen a rheumatologist to test for an autoimmune condition?

You might want to look into mesenteric Panniculitis. There are NIH articles online. Lots of doctors don’t know about it or dont know how to treat it. It can come and go. Best wishes!

Hi @soupful, and welcome to Mayo Clinic Connect! You have found a good place to share thoughts and encouragement with others. I have read helpful comments from @frouke @lylii @sagoodman and @cardigancorgi.

My own experience is that nothing takes the place of my lifestyle choices and for whatever reason, a high level of exercise and appropriate food choices at a super consistent pace helps my body function best. That in turn seems to help anything my doctors recommend work better. I would like to tell you that you will find that one cause, or a magic pill that works. Maybe you will. My experience has been doing all I can in that quest has made all the difference for me.

Diet, exercise, protecting the liver and managing other health conditions (like cholesterol, blood sugar, etc) are super important and make all the difference since the liver affects how the rest of the body functions.
