Time of day to take big 3

Posted by melb0606 @melb0606, Dec 18, 2024

Hi. I've seen a couple of people discuss the time of day they take each med of the big 3. Im about to start taking them daily and am trying to minimize GI issues. Cam you share when you take each mes during the day and if it is with or without food.
Thanks!

I have been on the Big 3 since May 2025, for MAC and pseudomonas. All was fine but I started feeling nauseas and losing weight, so my Dr told me to cut the Azithromycin and Ethambutol down a bit. I felt better after a week or so, but asked if I could take the full dose at night to try and deter the side effects. Does anyone have suggestion as to the time to take them? My Dr is on holidays for a month and I don't want to bother him.
Thank you.

Good morning.
I was on the "big" 3 for a long time. I managed by taking the ethambutol at night with a piece of toast before I went to bed. I slept through the side effects. That let me start the other 2 meds in the morning and spread them out during the day.....which lessened the side effects. I found if I was feeling nauseous, I purchased "sea bands" (bands that you put on the pressure points on your wrist) and they really helped. People use these on cruises. Depending on your dosage, my doctor gave me extended release tablets so it would enter my system slowly and I spread those out throughout the day (taking care to make sure if you had to time the pills before or after certain foods - e.g. calcium, dairy products, etc. I was on the meds for a long time and I was able to tolerate the regimen. Hope this helps.

I have learned through two separate periods of IV infusion therapy with three antibiotics, taking my meds immediately after a bland medium sized meal. I also have Zofran on board and Ginger Ale plus ginger chews available.
