Take Trazodone on a need basis

Posted by silmil93 @silmil93, Dec 22, 2025

Hello everyone,

I've just been prescribed Venlafaxine 75mg and started taking it on the 19th of December. Since then I have a terrible insomnia, I can barely sleep 3 hours per night. Prior to Venlafaxine I was talking clonazepam for panic attacks 2mg before bedtime, which I'm still taking but unfortunately it doesn't help with the insomnia caused by Venlafaxine.
My doctor suggested taking Trazodone on a need basis in addition to clonazepam 2 mg but I am wondering if that might cause some side effects as it's an antidepressant and usually antidepressants take up to 6 weeksto work. Would that be different for insomnia? Has anyone taken on a need basis? If so, what was your experience?
Thank you

@voja1953 that’s so interesting as Zoloft is generally considered sleep neutral (it causes sedation at the same rate it causes insomnia). Is it possible that you had an undiagnosed depression?

Every single night, or I’d never sleep again!

That's exactly how it would be. Luckily for me, a third psychiatrist saved me from insomnia.

I have taken Trazadone for years for sleep. I take it at night about an hour before I go to bed. I also take Cymbalta in the morning. If I take it at night, I am unable to sleep. Are you taking the Venalafaxine in the morning? I would think that the Trazadone along with the Clonazepam would knock you out. Trazadone is an old antidepressant and has been around a long time. I am not a doctor, but maybe start with 50 mg of the Trazadone for sleep and see if it helps. Please keep us posted. Thank you for bringing this up.

I do not understand the "as needed part" as if I wait to see if I need it, I have been in bed
for at least an hour & then if I take it, it will take at least an hour to kick in......AND...it
is to be taken with food, so I have to eat something at 10:00 o'clock at night....AND...
how much is one supposed to eat ???

Hmm, this is an interesting question. I am on Trazodone - which I take for pain. I had not heard it could be taken as on needed only? (I take mine twice daily.) I would appreciate your response.

@kayraymat I think generally the “as needed “ is to be determined an hour or two before bedtime. Are you sleeping at this point and don’t think you will it or amped up and think it will be difficult to sleep

Imo, Trazadone fits into a sleep schedule as an every night ritual for the reason you mentioned. Taking it as needed doesn’t mean one can’t take it every night, especially if on the nights you don’t take it you have insomnia and wind up taking it later

I meant to say sleepy, not sleeping

Gosh; I haven't felt sleepy in ages.
I have been taking it regularly for fear of not sleeping.
It is so comforting to know there is a medication that actually works.
Thanks for your explanation regarding "as needed." K

@worley725
I take a 50mg of Tramadol for pain each day in the early morning.
And sometimes I take 2 Tylenol for Arthritis if I will be walking a lot
as my left hip hurts & I also have Bursitis in my left knee.

I take 100 mg of Trazadone at night with/ after dinner in order to
sleep & it works 80-90% if the time. K

I had severe insomnia. Until I found a real third psychiatrist who saved me from insomnia and large doses of medication. Trazadone is a good but very strong antidepressant with a lot of side effects. My doctor recommended a dose of 50 mg before bed. No more. Other antidepressants are taken together only in the morning. Clonazepam is a difficult drug, you get used to it quickly, you need increasingly larger doses that knock you out - quitting is even harder. Try to take as few medications as possible. It is important to find the right combination, it is very important to monitor the effect and adjust the doses of the medication. Good luck!
