Starting IV Amikacin this week--Questions

I am supposed to start IV Amikacin this week. I don't know a lot of specifics. They put it in your arm, right? Will be dosed 3 times a week. Do you I administer the dose myself after the nurse sets me up? Does the nurse come back weekly to check and possibly insert a new IV? I have researched the side effects. It seems the worst is the effect on hearing. also Kidney can be affected and must be monitored. Any other major possible side effects? I have been putting this off for awhile but now have to start treatment other than Azithromycin and Ethambutol.
