Anyone familiar with the clinical trial RMC-6236? Or how to enroll?

Question,,,is anyone familiar with the clinical trial for RMC-6236? Or is currently enrolled in it? Or is familiar with how the process works?

Yes Naliri is similar to Folfirinox, but no Oxaliplatin, and the irinitocan is liposomal, supposed to stick to the cancer cells better than plain irinitocan. There is Naliri-Fox as well that does have the platinum based as well, but none of my doctors thought it was worth it on account of the neuropathy that I already have from Folfirinox. Yes I have to wear the pump for 46 hours.

There are three stages of trails starting with stage I which is dose escalation of the investigational new drug (IND) to determine the “therapeutic” dose to administer and in expanded phase III trials which often times are randomized, double blinded. The same trial does not have all three of these phases at the same time. When phase I finished and meets the objective, it goes to phase II. When the objectives are met successfully, it the is expanded to stage III.

It is possible that an IND can be used in different trials where it may be tested alone as monotherapy, in combination with another IND, or combined with a standard of care. This is the case with RMC-6236 and why you will find it in various phases. You will not find it standing alone in three different phases. You will have to see what is available in how it is configured and in what phase, research it for the benefits and risks in that phase and make an informed decision.

Thank you @stageivsurvivor When I was discussing with friend, I reached conclusion that they are probably testing it with another drug. Would it be possible to get a compassionate release of 6236 by itself? I'm hesitant (not that I have a lot of other options right now), about trying a new drug that's not proven. I inquired with my current oncologist about getting a compassionate release of Zolbetuximab and she said the rep says it's only for gastric cancer (in origin). Do you know about this drug for the cld18.2 protein; supposedly it's being reviewed by FDA now for pancreatic cancer. It seems it's only for HER2 mutations which I dont have. I'm only 80% positive for the cld18.2. Am I really out of luck as far as a compassionate release?

Marie I'm so sorry to hear of your problems! Can you look into getting on an RMC-6236 trial? It is systemic and all my doctors are recommending it. Do you know what kind of KRAS G12 mutation you have?

I am so disappointed to hear how Naliri has affected you.
You called it! Breathing problems.
I just took one week off to visit grandchild in California. My CA19-9 went from 63 to 100. However, the side effects this week have been minimal except some fatigue! I’m on day 5. Was able to work this weekend (I’m in real estate) and attended two social events in the evenings.
I think this disease is so systemic that we cannot let up the gas. Histotripsy and SBRT are more for interval tumor death of metastases and buy us time until the next thing.

I would be demanding to get back on what worked initially. Especially if there has been a long interval since doing it.

I did RM 9805 (KRAS G12D Target) with RM 6236 (pan KRAS). I am stage IV-ie its roaming around in my bloodstream. Ultimately I did have some progression, but I really think when it is combined with the right chemo it’s going to be a winner. Hopefully a resolution to the side effects to the skin have been resolved.
It worked for me for about 6 months. Great time to get my bloodwork back to normal and be ready for whatever could be next. For me, it’s Naliri right now. Maybe SBRT again.
Travelling for a second opinion this week. Best of luck with the trial. If I can help on the skin issues, don’t hesitate to reach out.

Wondering how much spread you had prior to starting the trial (if comfortable to disclose) or other factors that might have sffected the time it was effective. I have been on it for 10 months with no progression so am concerned how much longer I have before I see progression. It seems to be the best treatment out there for now.

Many have tried obtaining RMC-6236 for compassionate use. It is a drug not yet FDA approved and the developer is not providing it compassionate use. I know of several that inquired and supply is limited and needed for trial participants. I can imagine the company has been overwhelmed with requests but they are not able to fulfill those requests and ensure trial participants have adequate supply.

What about "the right to try"? I believe this was enacted sometime within the last 10 years. I do not know much else about it.

I was adamant to get into the trial last spring. My reoccurrence was in liver and was negligible but enough to get in!
A suspicious spot appeared on my lung and my spot at liver resection measured larger after 6 months so I chose not to continue. But I am very glad to have done it and hope it does not preclude me from future trials.
