Reclast Infusions: Side-effects & Recovery time

I just had a reclast infusion last week and have had serious side effects. I had the worst flue like aching for 5 days then my lefty arm became full of inflammation in the wrist, elbow and shoulder which caused extreme pain and I lost the ability to straighten my elbow. Ultrasound showed huge amounts of fluid throughout the arm. The right arm is now starting to have the same symptoms. The pain is excruciating. Has anyone else experienced anything like this? Neither the ER doctor nor the Dr. who prescribed the procedure knew what to do to ease the symptoms. Both arms from fingertips to shoulder are swollen and neither elbow will straighten. Anyone else have adverse reactions to the reclast infusion? If yes, how long did it last?

@judydav just curious if you are still experiencing symptoms from your Reclast infusion? It has been 18 months for me and there are days I can hardly move my right shoulder / arm and my left foot is almost impossible to walk on at times. These symptoms started the day after my first and last infusion. The pain and discomfort was so great it would wake me 5/6 times a night; now, maybe 1-2 a night; hopefully the day is coming that it will be gone. My endocrinologist would like to do additional treatment; I’ve decided against any additional treatment outside of having a retest of my bone density.

Sorry you have these problems ! I don't think I have problems but my left foot is full or arthritis. I am getting treatment for foot but ongoing. I had a DEXA test about 2 years ago and I was about the same. Having another DEXA scan in January. My primary doctor (of 25 years) took me off Fosamax and Boniva as had been taking for 10 years +. So he suggested Reclast as easy to use and only have every 2 years. For me never again. I have no problems from Prolia. Have you tried this? My arthritis doctor prescribed Hydroxychloroquine Sulfate and has helped the pain from my foot. Will be interested in seeing my DEXA results next month. What 'other
" tests' did your endo doctor want to do? I would look into due to your ongoing pain....just a thought !

I had a Revladt infusion several years ago. Infusion at noon. Woke up 3 AM next morning with serious shivering, but bed was wet from sweating. Felt 10 times worse than the worst flu I ever had. Balance was off, dizziness, severe headache. I told my endriconologist about my symptoms and she said those were highly unusual. However, I declined having any more treatments. I had tried all other orals - mostly gastric upset. Evista seemed most promising with no gastric problems. However, my migraines spiked - severe and frequent. Currently trying dietary and exercise routine changes.

Reclast (zoledronic acid) can cause pain, but not for everyone — and when it does, it’s usually temporary.
Here’s what most experience:
Common Types of Pain After a Reclast Infusion
Symptoms typically begin within 24–72 hours after the infusion and last 2–4 days (occasionally up to a week):
Muscle aches (like you’ve exercised hard)
Joint pain — especially hips, knees, shoulders
Bone pain — often described as “deep aching”
Headache
Back pain
This cluster is known as an acute phase reaction, and it happens because the medication stimulates your immune system temporarily.
How Common are they?
About 20–30% of first-time users get flu-like symptoms including body aches.
However…Pain is less common with second and third yearly infusions.
Most reactions are mild to moderate.
Who’s More Likely to Feel Pain?
1) First-time bisphosphonate users
2) People who are vitamin D deficient
3) Those who didn’t hydrate well beforehand

Ways to Reduce or Prevent Pain
Before and after the infusion:
Stay well-hydrated (2–3 glasses of water with electrolytes before and after)
Take acetaminophen or ibuprofen (if safe for you) starting right after the infusion and for the next 24 hours
Make sure your vitamin D is adequate (many doctors aim for ≥30 ng/mL)
Red Flags (Rare but important)
Contact your doctor if you have:
Severe pain that lasts more than a week
Fever above 101°F
Pain in the jaw or groin/thigh area that is persistent or worsening
Any signs of an allergic reaction (very rare)
Don’t know if you did any of these remedies…but may be helpful for others…the doctors are terrible at educating us on how to minimize or eliminate side effects…seems like they wait to see if you call in…and then they still don’t know…maybe they should have to try a dose😊

I agree with a lot that you expressed! Shouldn’t the endocrinologist have a check list before the infusion?
2–3 glasses of water with electrolytes before and after)
Take acetaminophen or ibuprofen (if safe for you) starting right after the infusion and for the next 24 hours
Make sure your vitamin D is adequate (many doctors aim for ≥30 ng/mL); check this Before, not after the infusion, make sure one is properly hydrated and if not, delay the infusion, have patients sit in waiting area, provide the necessary drinks with the electrolytes and possibly even provide the acetaminophen or ibuprofen (if safe for you) starting right after the infusion and for the next 24 hours; with a follow up call and / or visit to ensure the patient is not having severe symptoms from the infusion.
What I observed was; after the infusion, sever symptoms, expressed to endocrinologist, refers you to Rheumatologist, Neurologist or anyone else because the Reclast infusion did not cause your shoulder pain for the past 18 months! Well, yes it did and still does, just not as severe!

@judydav my doctor has set me up for another infusion which I have declined, did offer a different medication but I am not totally ready for any more issues. I have inquired about having a DEXA test to see what the Reclast might have done and go from there. I honestly dislike their saying, “ only one in a million people will have continued symptoms after their infusion “; must be a lot of millions!

It seems to me that most of the solutions are aimed at addressing the acute phase reaction while approaches to prevent/handle long term consequences are sorely missing.

I had 1 Reclast infusion (following Evenity) that caused my right knee to swell up, painfully. I couldn’t put weight on it for about a week. No prior knee issues.
My endocrinologist expressed surprise and didn’t really offer any solutions. I treated with RICE (rest, ice, compression & elevation) and it eventually resolved.
She’s recommended another Reclast, or Prolia which I have declined.
So far dexascan has been around 2.7 in lumbar spine which is stable. Exercising and continuing vit D/K2, and hoping for the best.
Might try Tymlos as it doesn’t stay in your body like the others.

I felt fluish for 4 days after my first Reclast infusion. After my second—which was infused after 15 minutes of a saline drip and 15 minutes after, I had almost no side effects. I also took Zyrtec before to counteract inflammation and three large glasses of water during the 3-4 hours before the infusion. I also continued with increased fluid intake for a couple of days.

I had severe muscle and joint pains and ran a temp. Felt like the worse flu that I ever had. I ended to getting sepsis shortly after (my immune system is shot and have had sepsis multiple times; 14 now. I've been on the reclast for the past several years. My last DEXA showed 8-11% increase in bone increase. My Dr now wants to change the prolia to Jubbonti. No explanation given

@rngma1118 if I were you, I would request an explanation! Remember, no one else is going to advocate for you or better than you can yourself. When I told my endocrinologist that I was not getting any more treatment he requested that I come in and go over the pros and cons of receiving some form of treatment. As we age our bones do become weaker / brittle perhaps. However, I would prefer to live a bit of a productive life for as long as I can without having to spend my days with aches and pains and consulting with my physical therapist on ways to help me recover from the side effects from a treatment that I had received. I’m not sure what brought me to having a bone density test that started all of this. I do recall being told years ago, “ If it is not broke, do not fix it”. I can and will research any and all future treatments I receive when and where possible!
