PVC in HOCM

Posted by selinac @selinac, Jul 31 11:09am

My daughter underwent myomectomy in 2020. Now she's 17. Had breathing difficulty. After MRI, discovered scarring in the LV of the heart muscles. Dr advised for ICD. Anyone had this problem before? What did you do? Is it high risks?

Hello @selinac, and welcome to Mayo Clinic Connect.
I am sorry to hear your daughter is having issues after all she's been through. It has to be frustrating and frightening at the same time.
I found this article online that is interesting, but I am not sure it answers the questions you asked

*Scarring in the left ventricle after a septal myectomy can occur as a result of the surgical procedure, which involves removing thickened heart tissue. This scarring may affect heart function, but many patients experience improved blood flow and reduced symptoms of hypertrophic cardiomyopathy following recovery.

Understanding Left Ventricle Scarring After Septal Myectomy
What is Septal Myectomy?
Septal myectomy is an open-heart surgery aimed at treating hypertrophic cardiomyopathy, a condition where the heart muscle thickens, particularly affecting the septum between the ventricles. During the procedure, a portion of the thickened septal tissue is removed to improve blood flow.

Causes of Left Ventricle Scarring
Scarring in the left ventricle can occur after any open-heart surgery, including septal myectomy. The primary causes include:

Surgical Trauma: The incision and manipulation of heart tissues can lead to scar formation.
Healing Process: As the heart heals, fibrous tissue may replace normal heart muscle, resulting in scarring.
Ischemia: Reduced blood flow during surgery can contribute to tissue damage and subsequent scarring.
Implications of Scarring
Left ventricle scarring can affect heart function in several ways:

Reduced Contractility: Scarring may impair the heart's ability to contract effectively.
Arrhythmias: Scar tissue can disrupt the electrical pathways in the heart, leading to irregular heartbeats.
Heart Failure: Significant scarring may contribute to heart failure symptoms over time.
Management and Monitoring
Post-surgery, patients should have regular follow-ups to monitor heart function. Key management strategies include:

Echocardiograms: To assess heart structure and function.
Medications: To manage symptoms and prevent complications.
Lifestyle Changes: Adopting a heart-healthy lifestyle can support recovery and overall heart health.
Understanding the potential for scarring and its implications is crucial for patients undergoing septal myectomy. Regular monitoring and proactive management can help mitigate risks associated with left ventricle scarring.

Is your daughter's cardiologist up to date on HOCM/HCM and post op septal myectomy conditions?
Have you considered a second opinion with a Center of Excellence such as the Mayo Clinic or Cleveland Clinic?
The Mayo Clinic is the world leader in Hypertrophic Cardiomyopathy and the see the most complicated cases that need an expert's input.

Hopefully another member who may be familiar with this same situation will respond, but in the meantime,
I would not hesitate to get another opinion. Your daughter has been through so much (and you as well as a mother!) and it's important to get the right answers.

I am sorry to read that your daughter is experiencing a complication from her septal myectomy. I did, too. Just 2 or 3 weeks after my septal myectomy in July 2022, I began experiencing dizziness and vertigo/giddiness. In October I had a syncope, wore a monitor/halter for a month, had another syncope (injuries each time), and no one could figure it out. When the monitor results were reported, I was experiencing arrhythmia when moving around. I was given a pacemaker/defibrillator, its name is Buddy, and it solved the problem 100%. The scar tissue is sometimes problematic, as the case here.

@karukgirl, thank you for sharing the article. It really helps.
You are right. We always seek second opinion.
We are in Malaysia and this case is rare in the hospitals in our country.
We are finding information and connection outside of our country. Need to be certain there are other solutions otherwise making a move to other place will be painful for her.
I heard of Mayo and Cleveland on HOCM. Cleveland asked for a fee for a visit there. Now trying to get if we could get more info from Mayo for a more solid response before we decide the next move.

Hi @walkinggirl Thank you for sharing.
Your arrhythmia was due to scarring in your heart or the heart function?
Hope you feel good now. May I know your age please?
How's the feeling after planting the device?

I am so glad you were able to get some help from the article.
I have posted Mayo Clinic international locations, perhaps there may be something closer?
Mayo Clinic in Rochester Minnesota is the world leader in treating HOCM/HCM.
But their experts may be able to work with other physicians internationally? I really don't know, but at least it's worth checking out.
I bet you are frantic trying to find help for your daughter! I hope you are taking care of yourself too. This must be a long journey for you and her.
Hang in there. Read as much as you can. Mayo Clinic Connect is here to offer support and listen!

Arrhythmia can be due to a problem in the electrical functioning of the heart. It is difficult to say if mine was due to the scarring or the heart function or a combination of the two. I was recovering from a septal myectomy and any heart surgery is a traumatic event for a person's heart. Scar tissue was forming as a part of the healing process. I will be 79 this month, I was almost 76 when I had surgery. I feel ABSOLUTELY GREAT and never had the dizziness and giddiness again after the ICD was installed. One of my cardiologists is a electrophysiologist, a specialist in the electrical functioning of hearts. The surgery to install the ICD was short, I went home the next day. The small incision healed quickly. I work out at the gym, walk at least 3-4 miles most days and do other physical activity with no problems.

Wow. You are really strong!! 💪

My daughter diagnosed with HOCM when she was born. She had her septal myectomy 5 years ago during the COVID time. She's turning 17 and has kind of emotional stress why does she have to go through this since young. We are hoping if there are easier solutions than planting the ICD nor pacemaker.
After the septal myectomy, any form of sharp objects, even a blood test, vaccination is a trauma for her.
