Propafenon 150 mg
I asked my cardiologist to prescribe me propafenon as a pill in the pocket for a-fib. I’ve only have had two or three episodes of a-fib since i had an ablation on in Feb . 2025. I don’t take any beta blockers (metropalol) since it lowers my heart rate to low. The cardologist prescribed me propafenon 150 mg 3 times a day. Do i need to take this if I’m always in normal sinus rhythm ?

In time, maybe....as your history progresses, or as your disorder progresses. Most should anticipate it coming back in time as most ablations have a finite life.
As to your prescription, which you say was meant for PIP, what induces your question about whether-or-not you should be taking it prophylactically....routinely, and not as a PIP? If you don't have confidence in the prescribed purpose, then you should really contact your physician and explain your concerns. PIP means 'as required'. You take one pill at the time of greatest need, that being the rare event of a bit of breakthrough AF....your description. The prescription was not for 'BID' or 'PO', meaning 'twice a day' and 'once a day' respectively.
Please pose this question to your cardiologist.

@bigj Eliquis (apixaban's brand name) is a direct-acting anti-coagulant. It's not like aspirin...it's better for the fibroid type of clot that is likely to form in the left atrial appendage (LAA). It is usually dosed, in adults, at 5 mg BID (twice daily). This is because it has a short half-life of about 10-12 hours or so, depending on how quickly one metabolizes it.
But, things change all the time. The latest research suggests that we don't need a DOAC all that often, especially if our episodes of AF are infrequent and of short duration...less than 12 hours or so. Different sources cite different numbers, so you should stick with what your advisors tell you.

1 Reaction@gloaming the propafenon pill in pocket is usually between 300- 500 milligrams. I don’t want to take a bunch of meds where I’m not in afib. One night i was at 120 bpm and took a 25 mg metropal tarante[ and i was in normal sinus rhythm in about twenty minutes.
@jmw823 Tell your prescriber you want it nmt 250 mg. This is the usual dose for adults weighing over 65 kg. Also, the 25 mg of metoprolol may have converted you inside of 20 minutes, but your heart may have been ready already to throw in the towel and revert to NSR. Not saying the metoprolol wasn't helpful, or instrumental, but your heart might have converted on its own ten minutes later. I never did revert to NSR when given metoprolol bolus. I was sent home and converted some hours later, long after the metoprolol had been reduced via metabolism.
