Pineapple and Tacrolimis?
Hi all,
Can kidney transplant patients taking Tacrolimis eat pineapple?
I know grapefruit, Seville oranges, and pomegranate are not good, but how about pineapple? Thanks!

@hello1234, I’ve never heard of avoiding pineapple. But I suppose it is possible and maybe another member has some information to share. I avoid fresh pineapple because it makes me itch.
Are you having a reaction that concerns you?
Hi @rosemarya 😊
I was watching TV and had a craving for some canned fruit. I went into the pantry and saw a can of pineapple. After I ate it, I got a panic feeling and couldn't remember if pineapple was on the interaction list with Tacrolimis. When I googled, there was no clear answer. So I did what I always do when I panic, I jumped on Mayo Connect to see what my fellow transplant team members do about pineapple?
Thank you Rosemary!!
I am a double lung tx recipient that takes Tacrolimus and Bromelain (pineapple extract for gastro issues). I have cleared it through my transplant team. When I was in the hospital, I ate pineapple with no issues or concerns. I also eat pineapple and drink pineapple tea regularly. My team knows about it and have never said that I need to stop. I am not sure about the kidney tx part of it though, but for the med there is no issues.
@hello1234 Here is an article from the National Kidney Foundation, which gives some good dietary guidelines. It also doesn't mention pineapple as an issue! https://www.kidney.org/kidney-topics/diet-after-kidney-transplant
As a CKD patient on dialysis, I do not drink grapefruit juice anymore, How disappointing, as it was the only thing to settle my stomach!
Ginger
Thank you so much @chickytina , @gingerw and @rosemarya ❤️.
It's very comforting to have my transplant family there when I need you!
Every once in a while my Tacrolimis blood level results bounce around from too high to high low. I always think it's probably something I ate or something I did. My target range is 6 to 8 and my last test result was 10.5.
I read on the internet that in addition to the NEVER EAT LIST (grapefruit, pomegranate, seville oranges); too much spinach,
banana, sweet potato, tomato
sauce, etc can cause a spike in the
TAC blood level. I never know what "too much" means. Do you eat half a banana or a sweet potato? Do you eat spaghetti and meatballs with marinara sauce? I get nervous about everything I eat.
It's very comforting to know that pineapple is something you eat (except for Rosemary and her itching response!)
My sister always says to keep a food log and then bring it to the doctor and that way you can see what might be effecting it. I don't do it, but she always tells everyone to do it regardless of the issue. This might be a good idea for you if you believe it might be something you are eating.
Due to the potassium in bananas and plantains, I can't eat them. I also can't have papaya. I usually have tomatoes at least once a week. I will make spinach from time to time, but have a lot of things (other than those things on the never eat list) in moderation,
Also included on the never eat list are: Salads made outside of the home, you don't know how well they wash the ingredients (I had E.coli that we believe came from salad). Food from salad bars, buffets or food trucks. Cold cuts and other high bacteria loaded items.
Thank you @chickytina ❤️ for your excellent and detailed food list. I appreciate it very much!
With tacrolimus, Pineapple is on my ‘good’ list and I eat lots, but pineapple juice is high in potassium.
Thank you so much @maggieinfp 😊 Your posting is very comforting to me.
It's so nice to meet you!
I am a four year kidney transplant patient. I had my transplant at Mayo Jacksonville.
How many years are you
celebrating with your new organ?
I am almost 5 months post transplant so still learning all the side effects of tacrolimus, but am doing well. Back on the golf course but missing my pool aerobics which they say never!
