PADCEV + KEYTRUDA: results and side effects

Posted by scbil @scbil, Apr 8 8:25am

My wife was diagnosed with bladder cancer January 2025. Due to age and physical condition she could not undergo bladder removal so underwent BCG and radiation/chemo treatments. Her bladder cancer was in remission but the cancer had metastasized stage 4 to her lungs. In January 26 she PADCEV AND KEYTRUDA TREATMENT with a repeating schedule of treatments for two weeks on and one week off. She has completed two series and been hospitalized following both. The first with a rash, sepsis any Ecoli and the second with dehydration, diarrhea and vomiting whenever she tries to eat or even smells food. Has anyone else had similar reactions to this treatment and, if so, did it improve or worsen?

Since January 2024 I have been treated for Stage 4 metastasized bladder cancer that has settled in the muscles walls of my abdomen. Initially treated with Cisplatin+Gemcitabine. Next came Carboplatin+Gemcitabine. Then
since August 2024 I have been treated with Trodelvy which has been quite successful, but some recent indications are that it is starting to lose some of its effectiveness, so we are most likely going to be switching to Keytruda+Padcev in the not too distant future.

However, there is concern about side effects on the eyes because in 2018 I had cornea transplants. So has anyone had side effect issues with their eyes attributable to the Keytruda+Padcev combination?

@jack404, that is a good question to also ask your prescribing oncologist. There might be ways you can ease the know vision and eye side effects of Keytruda+Padcev to not negatively impact the cornea transplant success.

You might find some helpful information in this discussion in the Gynecologic Cancer support group where members talk about eye effects of Keytruda alone:

When are you scheduled to start new treatment?
