PA vs Dr doing Biopsy

new here this am. Consult yesterday at Mayo. Surprised to hear the PA's do the Biopsy and remain the quarterback thru the Journey. I really liked the PA no problem there and the PA said I could have a Physician do it but any other experiences would be good to hear about. After reading other peoples stories my tumor seems small. It has a volume of .4 ml , had a Pi-rads score of 4, in left mid-gland peripheral zone.
Fusion perineal biopsy next week and I would prefer to do with no antibiotic to preserve my biome , PA wants to to do a 1 pill preventive antibiotic but I question that with less than half a % chance of infection . Has anyone else passed on the pre biopsy antibiotic ?

For what it's worth, I had fusion trans perineal biopsy done by NP at Mayo Rochester in August. No issues at all. She and her team did a great job from my experience.

You have a huge prostate and a small PSA in comparison. Have you been on antibiotics to try to resolve the BPH? That can frequently shrink the size of the prostate and relieve some of the tumor burden. I’ve heard of people going from over 100 cc down to 60 cc and having their PIRADS Scores that go down along with it.

A friend of ours had a huge prostate and a PSA of 50, but they never found any cancer in multiple biopsies.

That can frequently shrink the size of the prostate and relieve some of the tumor burden. I’ve heard of people going from over 100 cc down to 60 cc and having their PIRADS Scores that go down along with it.

A friend of ours had a huge prostate and a PSA of 50, but they never found any cancer in multiple biopsies.

No I have never tried the antibiotics to try and shrink my prostate. I will definitely bring that up to the urologist after my biopsy. I appreciate your feedback.

Do you think it could be asymptomatic chronic prostatitis ? My MRI didn’t mention seeing any infection.
Will the biopsy show it for sure if I have it ?

This is definitely something you need to talk to your urologist about.

Your prostate is quite large and that is not normal. It is usually due to some outside factor. I’ve seen people talk about how they are able to get their prostate size down by taking antibiotics because they have BPH. You need medical advice and I am not a doctor.

@dribbles you are NOT going to wipe out your intestinal flora with a single 500 mg Cipro - no way.
The antibiotic is given to prevent bacteremia - a blood infection which is usually transient (if at all) after biopsy. However, transperineal biopsies have about a Zero chance of this since the bacteria laden rectum is avoided completely.
I had no antibiotic administered after my TP biopsy. The worst part was hurriedly ripping the Band Aid off my taint the next morning when I realized it was still there just before a BM…OUCH!!!
