Mysterious shortness of breath: What has helped you?

I will try to make this as short as possible, but this has been going on for over 5 years, so it might be farily long. 

Beginning in summer of 2012, I began having shortness of breath (SOB) with no other symptoms. I felt a constant need to yawn, and every few breaths wouldn't satisfy the SOB. I would take a deep breath, and felt like it would get "stuck" before satisfying the air hunger feeling. About every 3-5 deep breaths would satisfy it, only for it to return a minute later. 

I got an endoscopy and other tests done, which revealed that I had some esophageal erosion due to acid reflux and a slight hiatal hernia and was diagnosed with GERD. I had always have bad heartburn, so I was prescribed with Prilosec, which I have been taking daily since them. I've tried stopping it a few times but the reflux always comes back a lot worse. 

Lung tests and x-rays were normal. Heart tests normal. Blood test revealed a slight anemia but otherwise pretty normal. 

I did some research reading forums where someone suggested taking vitamin B-12. Strangely, I took it and the SOB disappeared almost instantly. However, it only lasted a few days for it to return just as bad. I then started taking an iron supplement, which again made the SOB disappear quickly- same thing; symptom returned days later. 

After further research, I came across a breathing exercise method called the Buteyko method. Essentially you do a lot of breath holding to build up CO2 and reduce breathing as the theory is that I had chronic hyperventilation causing too much CO2 to exit my body. After applying the method and reducing my breathing, the SOB disappeared after only 2 days and I felt completely normal. I continued the method a few more days then no longer felt the need to pursue the exercises. I was normal for a whole year when the SOB once again returned with some chest tightness. I applied the method again and the symptom went away, this time with a little more effort; after about 3 weeks. I included physical exercise which also helped with my breathing. 

After that, I was normal for about 2 years. I mistakenly stopped or at least slowed down exercise and the SOB returned once again. I applied the method and began running for exercise but the SOB kept getting worse. It got so bad, I had multiple panic attacks and the feeling of completely empty lungs with the inability to satisfy it with deep breaths. I had to stop exercise altogether, apply the Buteyko method and do breathing exercises very carefully with very light and slow exercise. This helped, but it took many weeks for the SOB to improve. Then, it was almost normal when over a year ago as I was running, I couldn't get a deep breath to satisfy exercise-induced SOB. I have had SOB continuously since then (a year and a half). 

I once again started doing breathing exercises and slowly building up physical exercise, but I can't do any prolonged cardio activity because the SOB gets to a point where deep breathing will not satisfy it. While the breathing exercises have helped, they have had very little effect compared to previous efforts. It seems that every time the symptom returned, greater effort yields few results.

I suspect there is something, some underlying cause that is causing the SOB that has alluded me this entire time. 

So for the past few months to a year, the SOB is worse on some days, better on others, but never gone. There's no rhyme or reason or pattern for it. It's just there, sometimes affecting my sleep. I sometimes can't get a deep breath to satisfy it every now and then, but for the most part, a big gulp of air will satisfy it. But it returns seconds to minutes later. It's as though every breath doesn't deliver what it's supposed to, the SOB builds up, and then I have to take a big gulp of air to get rid of the feeling, pattern repeats. My breathing pattern is normal, however. I don't feel like anything physical is happening, but sometimes it feels like my airways and nostrils are slightly inflamed due to allergies, but when I don't feel inflammation the SOB is still there. 

Recent lung function tests show normal- I don't have asthma, or any other problems with my lungs. Heart tests are normal though I did have about a two week bout of heart palpitations which came and went. Haven't had any for a while- it just mysteriously started happening then stopped. Blood tests are normal, though tests always show a slight elevation of biliruben which my doc thinks is Gilbert's disease. 

I don't have sleep apnea (normal test), bloody oxygenation is normal, heart rate normal. 

I recently saw local naturopath (since mainstream docs aren't able to help) who immediately suspected a liver problem when I described my SOB, possibly liver inflammation. He used an electrodermal testing machine to test his theory which did seem to show a problem with my liver and gallbladder. He gave me digestive enzymes and a gallbladder formula to help clear a bile duct clog, thus reducing liver inflammation. He also determined with the machine that I have an egg sensitivity so I've been avoiding eggs. 

Been taking this and avoiding eggs for a couple of months, but there has been no noticeable improvement. Everything else is normal. Emotionally I'm normal- no anxiety, depression, etc. The SOB seems to be the only symptom of something, but always comes back worse, until a year and half ago when it came back and has remained since. I feel like I shouldn't have to do breath holding exercises every day just to maintain my breathing well enough to do every day things. 

Does anyone have any idea of a possible underlying cause?

2021/2022 UPDATE:
Since my original post about 3-1/2 years ago, a lot has happened, so I’d like to update the post to share with others who come across this what I’ve done since then.

I have maintained a weekly Buteyko method breathing exercise regimen where I do a few of these breathing exercises 3-4 times a week in the morning. This, in combination with daily light to moderate exercise (specifically weight lifting, with 1 or 2 days a week of walking and light jogging), I feel has kept the air hunger symptoms tolerable and manageable. I have mostly good days with some not-so-good days, but doing a breathing exercise and knowing that it’ll get better again helps me get through those times.

I also have sinus inflammation which can exacerbate the symptoms, but I’ve also managed this, which in turn reduces the severity of the air hunger symptoms.

So, while I haven’t found a cure nor is the problem completely gone, I have been able to maintain normalcy in daily life and manage the symptoms through the strategies I described.

This discussion remains active, alive and well through the comments section where others who have similar symptoms have shared what has helped them as well as suggestions for possible solutions to explore.

Since October 2020 it’s been pretty constant. My dr said it’s anxiety related but I don’t ‘feel’ anxious. I do have GERD (acid reflux). Along with Crohns Disease (remission 25 years), chronic kidney disease, chronic gastritis and gastroparesis. Lots of stomach issues. I did go to dr early in November and he did EKGand all was fine. I tried to do some belly breathing and that sometimes helps. But let me add I sleep fine. No trouble breathing at night. So from reading, this is normal?? Thanks for letting me share.

Hi @tonim. Glad you found us, not glad you’re going through this! Your situation seems to be common among us in this forum. It used to be that my breathing was fine at night but nowadays it does still affect me at night, but I think that has more to do with my sinus issues. One thing I am researching and will probably get tested for is vocal cord dysfunction. GERD can trigger symptoms and seems to be unaffected during sleep. This may not be the solution, but it always helps to eliminate conditions to narrow down to the answer.

Having shortness of breath is no way to spend vacations.

You have quite a few health ailments. The shortness of breath only happened while on vacation but since October you feel this way constantly? May I ask if anything has changed since October? For example, a recent job loss or any other major life changes.

Anxiety can cause a lot of physical symptoms but it's important to rule other potential causes of a symptom. It sounds like you are under proper medical care.

Do you see a therapist for anxiety? If so, have you considered doing so? If it is anxiety you can learn new skills to manage the symptoms. If it isn't anxiety you know to push for an alternative diagnosis.

Hello and thank you!! The SOB has been on and off since October. For example I’ve been good the past two days. My PCP put my on Zoloft and has increased to 75mg. But I don’t feel like I e been anxious. But dealing with this whole pandemic and being home 24/7 could do it too. Because of all my ailments yes I’m afraid to get COVID. and I did forget to mention I also have chronic kidney stones also. I will have to call my gastro dr soon. I do have an ileostomy (for 25 years) and have had diarrhea for about 2 weeks.
I would see a therapist. but I truly have a wonderful life. I don’t have anything complain about. I have an amazing support system between my husband family and friends.
So I love that I found this site and can talk ready what others are going through.

Yes and yes. This is my story. I can't believe you took the words right out of my mouth. I did buteyko as well. I have constant flare ups. Sometimes I'm fine for weeks. It's so mysterious. And I visited er many times when I thought I was choking. But vital signs were fine. I can't get a deep breath.

I will reply later. And a bit longer. I live in Europe. But your story kinda got me emotional since I visited er last time with the same symptoms and I feel like no one knows! It's been going on for a year now. I. 36 and female.

We have a LOT in common. I’ve had SOB for the last six months and was diagnosed with Persistent Asthma. I too generally sleep well at night (all the symptoms pretty much go away, so I REALLY look forward to bedtime). Anxiety is in play for me, though I didn’t realize it. My pulmonologist and PCP both suggested this to me without talking to each other. SOB and anxiety make for a toxic cocktail. It amazes me when I realize how tense and locked up I am without realizing it. I’m still dealing with excess phlegm, yet the breathing exercises help me. I absolutely have Asthma, but I think my brain exasperates it all. Lastly, I’ve too been home alone all day, every day (until my spouse gets home at 5:30) for about a year. This too has weighed on me.

I don’t feel anxious but I worry about everything!!! Though I do notice it more when I need to go out or around others. Afraid of getting Covid. And I’m the opposite, my husband works at home with me (same company) and he’s, let’s say, a little high strung lol! I would like some Alone time. WE DO EVERYTHING TOGETHER! I know I need to call my gastro dr. I’ve had constant diarrhea for two weeks. Thank goodness for this site!!!

I have ben experiencing this since september. I also have heart palpatations. I can not take a deep breath without effort. I have to be sitting down to do it. Or I have to stop to take the deep breath. According to my pulse ox it never goes below 95. I feel this a lot on exerction. I was told my heart is fine. Was told my lungs are fine as well. I think they are really taking me seriously. I am scared.

I have trace of regurgitation in 3 valves. Was told it does not cause symptoms. This is a daily thing. I can not seem to take a deep breath unless i am sitting down or stop doing what I am doing. I do not live anywhere near the mayo clinic or I would go there. Please soemoen help</p>

@tammyjbell You'll notice that I moved your question to a previous question where you asked the same question. I did this so you could connect with members in a central location. May I ask if you've had a chance to read all of the previous comments?

May I ask if either of you have been any major changes in your life around the time that these symptoms started?

Women tend to physically manifest anxiety physically and I'm wondering if anxiety could account for the symptoms.

@tonim You said you don't fee anxious but you worry about everything. Do you think worrying about everything is a type of anxiety? Did the worrying come before or after the symptoms?
